急性食物蛋白誘発胃腸炎(acute FPIES)の経口食物負荷試験(OFC)における,OFC施行時期,過去の症状の重症度や負荷量と,OFCでの誘発症状の重症度の関係については明らかでない.今回,生後1か月まで混合栄養で症状がなかったが,生後3か月時の普通ミルク再導入により軽症のacute FPIESを疑う症状を呈し,確定診断のために国際コンセンサスガイドラインに準拠した通常負荷量でOFCを施行したところ,意識障害やアシドーシスを伴う重症な症状を呈したacute FPIESの乳児例を経験したため報告する.乳児期,また最終エピソードからOFCまでの期間が短い場合は,ガイドラインに準拠した負荷量でも重症の誘発症状を生じる可能性があり,負荷量設定,緊急時対応の事前準備が,安全なOFC実施に重要である.Acute FPIESに対するOFCの方法はまだ標準化されておらず,今後のエビデンスの蓄積が必要である.
