We report two cases cf acute renal failure in transplant patients using cyclosporine-A (CsA) after the introduction of angiotensin-converting enzyme inhibitor (ACEI) to control arterial hypertension. They had no artery stenosis or rejection. Both presented severe tubular necrosis (ATN), which subsided discontinuation ACEI. Synergistic toxic effect ACE1 and CsA on tubules might explain ATN these cases.
