症例は65歳の男性で,2005年より閉塞性動脈硬化症による高位大動脈閉塞にて当院血管外科外来に通院中であった.2009年,便潜血陽性を主訴に当院内科を受診した.大腸内視鏡でS状結腸に3 cm大のIspポリープを認め,腺腫内癌のSM浸潤が疑われたため,手術目的に当科紹介となった.CTでは,腫瘍は周囲への浸潤が見られず,病的腫大リンパ節や他臓器転移も認めなかった(c stage I).また,血管再構成にて下腸間膜動脈の血流は,中結腸動脈左枝からRiolan動脈弧を経由し,左結腸動脈を介して供給されていた.これにより,血流障害を回避するため,D1郭清を伴うS状結腸切除を行った.今回,我々は高位大動脈閉塞を合併したS状結腸癌に対し,術前のCTによる血流評価によって,安全に手術を施行しえた症例を経験したので,文献的考察を加えて報告する.
