症例は84歳男性.55年来の2型糖尿病でインスリン療法が行われていた.胆道系腺癌を認め,胆管ステントが留置されていた.2週間前より発熱と食思不振が出現しインスリン療法を自己中断した.2日前より意識障害(JCSII-20)が出現し,救急搬送された.血糖値632 mg/dL,血漿浸透圧391 mOsm/kg・H2O,血清Na 163 mEq/L,尿ケトン体±より,高浸透圧高血糖状態と診断し,敗血症の合併もあり,緊急入院した.補液及びインスリン投与,抗生剤投与によって血糖値や血清Na値は緩徐に改善した.意識障害も改善したが,入院第3病日の食事再開の際に嚥下困難を認め,嚥下内視鏡検査で嚥下反射遅延と唾液の咽頭貯留を指摘された.頭部MRIでは橋正中部のT2強調像高信号域を認めており,浸透圧性脱髄症候群と考えられた.嚥下リハビリテーションによって経口摂取が可能となるまで回復した.高浸透圧高血糖症候群に浸透圧性脱髄症候群を合併することは比較的稀である.さらに本症例では嚥下機能障害が主症状であり,他疾患との鑑別に困難を要した貴重な症例と考えられたため報告する.
