ABSTRACT Galactomannan detection in bronchoalveolar lavage (BAL) fluid samples (GM test) is currently considered the gold standard test for diagnosing invasive pulmonary aspergillosis (IPA). The limitations, however, are various turnaround times and availability of testing. We compared performance GM testing with that conventional culture, an Aspergillus lateral-flow-device (LFD) test, a beta- d -glucan (BDG) PCR assay by using BAL from immunocompromised patients. A total 78 patients at risk...

Few studies have thoroughly investigated the causes of kidney graft loss (GL), despite its importance.A novel approach assigns each persistent and relevant decline in renal function over lifetime a allograft to standardized category, hypothesizing that singular or multiple events finally lead GL. An adjudication committee three physicians retrospectively evaluated indication biopsies, laboratory testing, medical history all 303 GLs among 1642 recipients transplants between January 1, 1997...

A total of 10/14 (71%) IPA resulted positive new LFD, including 8/9 true-positive 2/5 false-negative results old LFD. All without negative LFD; specificity was significantly improved

Early diagnosis of invasive aspergillosis (IA) remains challenging, with available diagnostics being limited by inadequate sensitivities and specificities. Triacetylfusarinine C, a fungal siderophore that has been shown to accumulate in urine animal models, is potential new biomarker for IA.

Summary Data on diagnostic performance of Galactomannan ( GM ) testing in patients under mould‐active regimens are limited. Whether sensitivity for diagnosing breakthrough invasive aspergillosis (IA) is decreased antifungal prophylaxis/therapy remains therefore a point discussion. Patient care after kidney transplantation requires integration of complex information to make informed decisions on risk constellations. Many machine learning models have been developed for detecting patient outcomes in the past years. However, performance metrics alone do not determine practical utility. We present a newly clinical decision support system (CDSS) detection patients at rejection and death-censored graft failure. The CDSS is based routine data including 1,516 transplant...
