Advanced generation ablation technologies have been developed to achieve more effective pulmonary vein isolation (PVI) and minimize arrhythmia recurrence after atrial fibrillation (AF) ablation. We randomly assigned 346 patients with drug-refractory paroxysmal AF contact force-guided radiofrequency (CF-RF; n=115), 4-minute cryoballoon (Cryo-4; or 2-minute (Cryo-2; n=116). Follow-up was 12 months. The primary outcome time first documented of symptomatic asymptomatic tachyarrhythmia (AF,...

Etripamil is a fast-acting, intranasally administered calcium-channel blocker in development for on-demand therapy outside health-care setting paroxysmal supraventricular tachycardia. We aimed to evaluate the efficacy and safety of etripamil 70 mg nasal spray using symptom-prompted, repeat-dose regimen acute conversion atrioventricular-nodal-dependent tachycardia sinus rhythm within 30 min. RAPID was multicentre, randomised, placebo-controlled, event-driven trial, conducted at 160 sites North...

Abstract The anti‐inflammatory actions of the mitochondrial peripheral benzodiazepine receptor (PBR) agonist PK11195 [1‐(2‐chloro‐ phenyl)‐ N ‐methyl‐ N‐ (1‐methylpropyl)‐3‐isoquinoline‐carboxamide] were investigated in human microglia. Application microglial inflammatory stimulus lipopolysaccharide (LPS, at 100 ng/mL for 3 h), induced enhancement expressions inducible enzyme, cyclooxygenase‐2 (COX‐2) and pro‐inflammatory cytokine, tumor necrosis factor‐α (TNF‐α). (at 50 µ m ) significantly...

There is a relative paucity of data linking inappropriate implantable cardioverter-defibrillator (ICD) shocks to adverse clinical outcomes. To examine the association between ICD and mortality or heart transplantation in large population cohort.A cohort study which included all subjects who underwent implantation 1998 2008 were followed up at our institution.Multivariable Cox regression analyses conducted investigate effect on risk death transplantation. Appropriate therapies modelled as...

Patients with Ebstein anomaly are known to have a higher incidence of interatrial communications and shunting blood its components through, mainly due either streaming tricuspid regurgitation or elevated right atrial pressure. Here we describe case where permanent pacemaker lead kept patent foramen ovale open leading right-to-left exertional hypoxemia. This is the first such report in published literature.

Abstract The effects of extracellular acidification on Ca 2+ ‐dependent signaling pathways in human microglia were investigated using ‐sensitive fluorescence microscopy. Adenosine triphosphate (ATP) was used to elicit responses primarily dependent the depletion intracellular endoplasmic reticulum (ER) stores, while platelet‐activating factor (PAF) store‐operated channel (SOC) influx . duration transient induced by ATP not significantly different standard physiological pH 7.4 (mean 30.2 ± 2.5...

Evidence regarding the incremental benefit of cardiac resynchronization therapy (CRT) with a defibrillator (CRT-D) versus without (CRT-P) in elderly patients heart failure is limited. We compared mortality and hospitalisation between CRT-D CRT-P elderly.A retrospective chart review identified all consecutive age ≥75 CRT implantation over last 10 years at Canadian tertiary care centre. Kaplan-Meier survival analyses cumulative incidence curves were used to compare time first hospitalisation,...

Takotsubo cardiomyopathy (TTC) may clinically mimic an acute coronary syndrome and is most often associated with stressor. In this case report, we describe atypical of TTC triggered by multiple subacute stressors rather than 1 We also report transient global amnesia occurring concurrently. This highlights that can present without a typical stressor should nonetheless considered in the appropriate clinical context. Concomitant presentation uncommon, but result from 2 conditions potentially...

We report an unusual complication of permanent pacemaker (PPM) implantation in a patient with temporary transvenous (TVP) situ, which the TVP lead formed knot around newly-placed right ventricular (RV) lead, complicating removal. The case underscores fact that suboptimal placement, such as looping RV, can result complications during pacemaker.Wiens EJ, Seifer CM et al. Marfan syndrome is associated with a high incidence of aortic root aneurysm and life-threatening dissection. With the successful use surgical replacement, dissection-related mortality has been significantly reduced. We present case patient who presented heart failure secondary to an unusual graft-related complication 14 years after Bentall procedure. Investigations revealed supra-aortic stenosis resulting from kink in graft caused by pressure expanding arch aneurysm. The underwent surgery...

An 18 year old male with an incompletely healed clavicle fracture presented unexplained syncope. Subsequent investigations were consistent a diagnosis of arrhythmogenic right ventricular cardiomyopathy (ARVC). A subcutaneous implantable cardioverter-defibrillator (S-ICD) was successfully implanted and defibrillation threshold (DFT) testing performed as per standard protocol. Shortly following the procedure, patient complained pain swelling over left clavicle. radiograph revealed aggravation...
