症例は62 歳女性.56 歳時に糖尿病性腎症から末期腎不全に至り血液透析を導入された.右手関節近くで橈骨動脈と橈側皮静脈が側々吻合された内シャントにより血液透析を施行されていた.数カ月前より,右手の腫脹や手背静脈拍動を認めるようになり,精査目的で前医より当院紹介受診となった.血管エコー検査では,吻合部近傍の橈側皮静脈の中枢側でシャント静脈は限局的に閉塞していた.そのため,側々吻合された橈側皮静脈の末梢側へ動脈血流が逆行性に灌流することで手背静脈網の怒張を来し,末梢静脈高血圧症により手が腫脹していると考えられた.動静脈吻合部に問題はなく,シャント閉塞部位は限局しており,閉塞部より中枢側の橈側皮静脈の性状は良好であった.手術は,吻合部近辺から手背にかけて,拡張した橈側皮静脈を剝離し授動化,末梢側を離断し反転させ,閉塞部位より中枢側の橈側皮静脈に端々吻合することで,シャントを再建することができた.
