Treatment of pleural infection with instillation intrapleural tissue plasminogen activator (tPA) and human recombinant DNase (DNase) has been proven to decrease the length hospital stay, surgical referral, improve drainage. The optimal dosage, administration, timing, frequency regimen remain unclear. It is unknown if two drugs can be administered immediately one after other (referred as concurrent) instead instilling them separately a 1- -2-hour interval in between. To assess safety efficacy...
Rationale: There is a paucity of evidence regarding the role tunneled pleural catheters in effusions caused by congestive heart failure that refractory to medical management. Objectives: The aim this study was assess feasibility catheter drainage for treatment associated with failure, either when used alone or concomitant talc pleurodesis performed during thoracoscopy. Methods: This retrospective cohort study. We identified patients and recurrent symptomatic who were treated between 2005 2015...
Background: Interstitial lung diseases (ILD) unresponsive to medical therapy often require transplantation (LTx), which prolongs quality of life and survival. Ideal timing for referral LTx remains challenging, with late associated significant morbidity mortality. Among other criteria, patients ILD should be considered if forced vital capacity (FVC) is less than 80% or diffusion carbon monoxide (DLCO) 40%. However, data on rates are lacking. Objectives: To evaluate based pulmonary function...
AimTo explore the feasibility and accuracy of virtual reality (VR) derived from cardiac computed angiography (CCTA) data to predict left atrial appendage occlusion (LAAO) device size.MethodRetrospective patients who underwent LAAO according clinical indication were reviewed; all a pre-procedural CCTA. Measurements (LAA) orifice diameters by CCTA, VR, transesophageal echocardiography (TEE) (acquired during procedure) compared implanted size. The LAA perimeter was calculated using Ramanujan...
Type 2 diabetes mellitus (T2DM) is a metabolic disorder characterized by raised blood glucose levels and peripheral insulin resistance. It an increasingly prevalent global healthcare concern. Conventional treatment options are limited in this context, there renewed interest evaluating the clinical biological effects of traditional therapies. We assess effect new herbal composition SR2004 on hemoglobin A1c (HbA1c), fasting glucose, lipid profiles patients with T2DM.This single center,...

Rationale: Cryodebridement (CD) refers to the removal of obstructive material from lumen tracheobronchial tree by freezing with a cryoprobe, which is usually inserted through flexible bronchoscope. This method achieving instant recanalization airways has been established for over 20 years, but published experience comprises limited case series. Objectives: study describes single large-volume referral center experience, including clinical outcomes and safety profile. Methods: Electronic medical...
Postoperative muscle pains occurred in 16% of 25 patients given 10 mg diazepam IV 5 minutes prior to succinylcholine. 60% not before The difference is statistically significant. Diazepam reduced the severity and duration postoperative pains, as well their frequency.
Anatomic changes may occur during chemoradiation treatment for lung cancers, requiring adaptive replanning. Here we characterize these cases.
Abstract We report 8 cases of persistent severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) pneumonia in patients previously treated with anti-CD20 monoclonal antibodies. Polymerase chain reaction nasopharyngeal swabs for SARS-CoV-2 was negative most cases; viral cell cultures confirmed that viable SARS-Co-2 virus present. Four were anti-SARS-CoV-2 hyperimmune globulins rapid resolution disease.
The effects of calcium gluconate pretreatment on succinylcholine-induced postoperative muscle pain and the changes in serum potassium levels were studied 80 patients undergoing elective surgery who to become ambulatory same evening. A test group 40 was given 10 ml 10% intravenously immediately before induction anesthesia with thiopental followed by succinylcholine for tracheal intubation. control without gluconate. Muscle pains graded according severity as nil, mild, moderate, severe....

Rationale: Exudative pleural effusions may arise secondary to inflammation of intra-abdominal structures. Pleural space loculations can complicate these effusions, preventing adequate chest tube drainage and leading consideration surgical intervention. Previous studies have demonstrated that intrapleural administration tissue plasminogen activator (tPA) combined with human recombinant DNase improve fluid reduce surgery for patients loculated parapneumonic effusions; however, the efficacy...
Objective The objective of the current study was to determine gas exchange abnormalities and physiological changes among healthcare workers during a 4-hour emergency department (ED) shift while wearing N95 respirator. Methods Single-center prospective observational study. Comparisons paired measurements were performed using non-parametric Wilcoxon matched-pairs signed-rank test. Results: Forty-one subjects included. Prolonged respirator use associated with significant decline in plasma pH...
Summary A case of glottic valvular obstruction following extubation is reported. The cause was bilateral dislocation arytenoidr due to coughing on the tracheal tube, which resolved automatically a further bout coughing.

Lignocaine pretreatment (2 mg/kg) significantly restricted the increase in serum potassium and decrease calcium caused by suxamethonium. Suxamethonium muscle pains occurred only 8% of patients who received lignocaine just before induction anaesthesia. The incidence was 45% those were not given lignocaine.
Objective: Intensity-modulated radiotherapy (IMRT) has better normal-tissue sparing compared with 3-dimensional conformal radiation (3DCRT). We sought to assess the impact of technique on pathological and clinical outcomes in locally advanced non-small cell lung cancer (LANSCLC) treated a trimodality strategy. Methods: Retrospective review LANSCLC patients from August 2012 2018 at Sheba Medical Center, Israel. The strategy consisted concomitant chemoradiation 60 Gray (Gy) followed by...
Severe respiratory failure caused by COVID-19 often requires mechanical ventilation, including extracorporeal membrane oxygenation (ECMO). In rare cases, lung transplantation (LTx) may be considered as a last resort. However, uncertainties remain about patient selection and optimal timing for referral listing. This retrospective study analyzed patients with severe who were supported veno-venous ECMO listed LTx between July 2020 June 2022. Out of the 20 in population, four underwent excluded....
