In this comparative biomarker study, we analysed 1768 serial sputum samples from 178 patients at 4 sites in Southeast Africa. We show that tuberculosis Molecular Bacterial Load Assay (TB-MBLA) reduces time-to-TB-bacillary-load-result days/weeks by culture to hours and detects early patient treatment response. By day 14 of treatment, 5% had cleared bacillary load zero, rising 58% 12th week treatment. Fall correlated with mycobacterial growth indicator tube time-to-positivity (Spearmans...
BackgroundChildhood tuberculosis remains a major cause of morbidity and mortality in part due to missed diagnosis. Diagnostic methods with enhanced sensitivity using easy-to-obtain specimens are needed. We aimed assess the diagnostic accuracy Cepheid Mycobacterium Host Response prototype cartridge (MTB-HR), candidate test measuring three-gene transcriptomic signature from fingerstick blood, children presumptive disease.MethodsRaPaed-TB was prospective study conducted at four sites African...
Mycobacterium tuberculosis complex (MTBC) Lineage 3 (L3) strains are abundant in world regions with the highest burden. To investigate population structure and global diversity of this major lineage, we analyzed a dataset comprising 2682 L3 from 38 countries over 5 continents, by employing 24-loci mycobacterial interspersed repetitive unit-variable number tandem repeats genotyping (MIRU-VNTR) drug susceptibility testing. We further combined whole-genome sequencing (WGS) phylogeographic...
On 9 January 2015, in a rural town Mozambique, >230 persons became sick and 75 died of an illness linked to drinking pombe, traditional alcoholic beverage. An investigation was conducted identify case patients determine the cause outbreak. A patient defined as any resident Chitima who developed new or unexplained neurologic, gastrointestinal, cardiovascular symptom from at 6:00 am through 12 11:59 pm. We medical record reviews, healthcare worker community surveys, anthropologic toxicologic...
Mozambique is one of the countries with highest burden tuberculosis (TB) in Sub-Saharan Africa, and information on predominant genotypes Mycobacterium circulating country are important to better understand epidemic. This study determined strain lineages that cause TB Mozambique. A total 445 M. isolates from seven different provinces were characterized by spoligotyping resulting profiles compared international database SITVIT2. The four most observed were: Latin-American Mediterranean (LAM, n...
Abstract Background The World Health Organization–approved Xpert MTB/XDR test detects Mycobacterium tuberculosis and resistance to isoniazid, fluoroquinolones, ethionamide, injectable drugs directly in specimens. This pragmatic, laboratory-based study assessed the diagnostic accuracy feasibility of a reflex testing approach, where was performed on residual specimens previously processed for MTB/RIF Ultra. Methods Routine respiratory specimens, Ultra, were stored sample reagent buffer at...
Multiple-locus variable-number tandem repeat analysis (MLVA) is useful to establish transmission routes and sources of infections for various microorganisms including Mycobacterium tuberculosis complex (MTC). The recently released SITVITWEB database contains 12-loci Mycobacterial Interspersed Repetitive Units – Variable Number Tandem DNA Repeats (MIRU-VNTR) profiles spoligotype patterns thousands MTC strains; it uses MIRU International Types (MIT) Spoligotype (SIT) designate clustered...
The Beijing genotype is a lineage of Mycobacterium tuberculosis that distributed worldwide and responsible for large epidemics, associated with multidrug-resistance. However, its distribution in Africa less understood due to the lack data. Our aim was investigate prevalence possible transmission strains Mozambique by multivariate analysis genotypic, geographic demographic A total 543 M. isolates from were spoligotyped. Of these, 33 lineage. genetic relationship between studied identification...
Tuberculosis (TB) remains a major health problem within the Community of Portuguese Language Speaking Countries (CPLP). Despite marked variation in TB incidence across its member-states and continued human migratory flux between countries, considerable gap knowledge on Mycobacterium tuberculosis population structure strain circulation countries still exists. To address this, we have assembled analysed largest CPLP M. molecular drug susceptibility dataset, comprised by total 1447 clinical...
Background As part of ongoing efforts to generate evidence needed on HIV and tuberculosis (TB) inform policies programs aimed improve the health outcomes migrants communities affected by migration mining, a preliminary investigation was conducted through biological behavioral (BBS) approach related TB in two origin migrant mineworkers Gaza Province. The main objective determine prevalence rates asymptomatic infection TB, social risk factors associated. Methods A cross-sectional survey from...
Mozambique reported the first case of coronavirus disease 2019 (COVID-19) in March 2020 and it has since spread to all provinces country. To investigate introductions SARS-CoV-2 Mozambique, 1 142 whole genome sequences sampled within were phylogenetically analyzed against a globally representative set, reflecting 25 months epidemic. The epidemic country was marked by four waves infection, associated with B.1 ancestral lineages, while Beta, Delta, Omicron Variants Concern (VOCs) responsible...
Tuberculosis (TB) is an important cause of morbidity and mortality among people living with HIV (PLHIV). Current WHO-recommended strategies for diagnosing TB hospitalized PLHIV rely on symptom screening disease severity to assess eligibility urine lipoarabinomannan lateral flow (LF-LAM) molecular testing. Despite these recommendations, autopsy studies show a large burden undiagnosed admitted PLHIV. The EXULTANT trial aims the impact expanded strategy using three specimens (sputum, stool,...
The World Health Organization's 2035 vision is to reduce tuberculosis (TB) associated mortality by 95%. While low-burden, well-equipped industrialised economies can expect see this goal achieved, it challenging in the low- and middle-income countries that bear highest burden of TB. Inadequate diagnosis leads inappropriate treatment poor clinical outcomes. roll-out Xpert(®) MTB/RIF assay has demonstrated molecular diagnostics produce rapid initiation. Strong services are still limited...
(1) Background: Laboratory-based molecular assays are the gold standard to detect SARS-CoV-2. In resource-limited settings, implementation of these has been hampered by operational challenges and long turnaround times. Rapid antigen detection tests an attractive alternative. Our aim is evaluate clinical performance two SARS-CoV-2 rapid during a high transmission period. (2) Methods: A total 1277 patients seeking diagnosis were enrolled at four health facilities. Nasopharyngeal swabs for real...
We evaluate the performance of TBcID assay in a panel 100 acid-fast bacilli cultures. Sixty-four isolates were positive for Mycobacterium tuberculosis complex (MTBC), whereas 36 gave negative results. These included 28 nontuberculous mycobacteria, one nonmycobacterial isolate, M. tuberculosis, and six bovis BCG strains. This corresponds to sensitivity 90.14%, specificity 100%, predictive values 100% 80.55%, respectively. The test is rapid, easy perform interpret, does not require sample...
"Ancestral" Mycobacterium tuberculosis complex (MTBC) strains of Lineage 1 (L1, East African Indian) are a prominent (TB) cause in countries around the Indian Ocean. However, pathobiology L1 is insufficiently characterized. Here, we used whole genome sequencing (WGS) 312 from 43 to perform characterization global population structure and correlate this analysis synthesis phenolic glycolipids (PGL) - known MTBC polyketide-derived virulence factors. Our results reveal presence eight major...
Background Tuberculosis (TB) is a difficult-to-treat disease requiring the combination of four antibiotics for minimum 6 months. Rapid and quantitative biomarkers to monitor treatment response are urgently needed individual patient management clinical trials. C-reactive protein (CRP) often used clinically as rapid marker inflammation caused by infection. We assessed relationship TB bacillary load CRP response. Methods Xpert MTB/RIF-confirmed pulmonary cases were enrolled assessment in...
Background: In resource-poor countries, antigen-based rapid tests (Ag-RDTs) performed at primary healthcare and community settings improved access to SARS-CoV-2 diagnostics. However, the technical skills biosafety requirements inherent nasopharyngeal oropharyngeal (OP) specimens limit scale-up of testing. The collection nasal-swabs is programmatically viable, but its performance has not been evaluated in settings. Methods: We first SteriPack self-collected nasal swabs for detection by...
ABSTRACT Dried blood spot (DBS) may overestimate the viral RNA, mainly in patients with low load (VL), due to proviral DNA and intracellular RNA.
