We report a rare case of primary synovial sarcoma the pericardium. Surgical resection was incomplete and chemotherapy radiotherapy (tomotherapy) were therefore administered. Regular cardiac magnetic resonance imaging scans used throughout follow-up, allowing effective planning application adjuvant therapies.
