Abstract A healthy 42‐year‐old male presented with a 1‐week history of pain associated swelling localised to the upper right quadrant his mouth. The patient had undergone bimaxillary osteotomy 20 years previously. An initial clinical diagnosis included an inflammatory or developmental odontogenic cyst leading enucleation lesion. Post‐operative histopathology revealed surgical ciliated maxilla. This case highlights need for awareness this ensure appropriate management.
