26歳女性, 米国人. 1年前にALT (Assistant Language Teacher) として来日した. 頭痛と嘔気で前医を受診し, 急性腎不全, 高血圧症, 血小板減少と診断され入院した. 次第に腎機能が増悪し, 当院へ転院した. 前腕の皮膚硬化やRaynaud症状から強皮症と診断した. 急性腎不全の原因は強皮症腎クリーゼと診断し, ACE阻害薬を中心とした降圧療法を行ったが, 末期腎不全へ至った. 米国へ帰国して, 腎代替療法を導入することを提案したが, 患者は日本で仕事を続けることを強く希望したため, 日本で腹膜透析を導入した. 就労期限の後, 米国へ帰国した. 貴重な症例であり, 腹膜透析の導入から帰国に至るまでの経過を報告する.
