We describe three cases of women with hot bath-related headache who reported that their severe paroxysmal could be provoked by pouring water over themselves or soaking in a bath. In one patient, the was also brought on exposure to cold wind. Another patient had headaches after she dived into pool and started swimming. 失語で発症したBasedow病とモヤモヤ病の合併例 (35歳女性) を報告した.自発語の減少を主訴として受診し, 軽い意識障害 (JCS1), 超皮質性運動失語, 軽度の右片麻痺を示した.同時に, 甲状腺機能亢進が発見され, 抗甲状腺剤の投与後, 甲状腺機能の正常化につれて神経症状が改善した.頭部単純X線CT, 頭部MRIにて脳梗塞が確認され, 脳血管撮影によりモヤモヤ病と診断された.本症例においてモヤモヤ病の症状発現には, 甲状腺機能亢進症による脳血流量の減少が関与した可能性がある.また, モヤモヤ病とBasedow病の発生機序における関わりも示唆される.

患者63歳女性. 両側前腕の腫脹, 四肢の筋力低下で発症. 臨床症状, 血清CK高値等の検査所見より, 多発筋炎と診断した. プレドニゾロン内服により多発筋炎が寛解した後, 数カ月して両側眼瞼下垂, 複視が出現した. エドロフォニウム試験陽性, 抗AchR抗体高値で重症筋無力症と診断した. 胸部CTで, 前縦隔に石灰化を伴う胸腺腫を認めた. 拡大胸腺摘除術にて重症筋無力症症状は改善した. 多発筋炎に重症筋無力症が合併することは極めて稀であるが, 多発筋炎治療中に症状の増悪を認めた場合には重症筋無力症の合併を考慮する必要がある. さらに合併例では胸腺腫が比較的高頻度に認められることから, 胸腺腫による自己免疫異常の両疾患への関与が示唆される.

症例は72歳, 女性. 65歳時に脳動脈瘤破裂の既往がある. 約2年間, 本態性振戦の診断のもと trihexyphenidyl HCl (以下THPと略す) 6mg/日を近医より投与されていた. その後, 患者が抑うつ状態に陥ったため三環系抗うつ剤 dosulepin (以下DLと略す) が追加投与された. DL投与開始から約2週間後, 起床時より, 頸部, 眼球, 舌, 口唇に不随意運動が出現した. 不随意運動は1カ月以上持続したが, THP, ついでDL投与中止で徐々に軽減, 約2週間でほぼ消失した. また, THP再投与により増悪した. 本症例はTHP, DLの抗コリン作用が重複し, 線条体アセチルコリン, ドーパミン作動系の機能バランスがくずれたため不随意運動が生じたと考えられた.
