We investigated the prevalence of porcine circovirus 2 (PCV2) and 3 (PCV3) in wild boars captured or found dead Nagano Prefecture 2020. Based on PCR testing, 21 254 (8.3%) were PCV2-positive 43 256 (16.8%) PCV3-positive, 5 253 (2.0%) both PCV3-positive. 一般に黄連湯を皮膚疾患に対して使用することは稀である。今回,「上熱中寒」を参考にして,黄連湯を顔面紅斑 ・紅潮に用い,有効であった5例を経験した。「上熱」や「中寒」に関して,具体的な症候はこれまであまり報告されていないが,「温まると悪化する顔面紅斑・紅潮」「赤ら顔」「ほてり」「のぼせ」「舌の黄苔」を「上熱」,「温かい飲み物を好む」「冷たい飲み物で下痢が悪化する」「他覚的な心下の冷感」を「中寒」と考えることで,黄連湯を皮膚疾患に応用することが可能であった。
Bovine leukaemia virus (BLV)-infected Holstein cattle carrying certain bovine leukocyte antigen (BoLA)-DRB3 alleles were previously shown to be resistant BLV provirus multiplication, while those other susceptible. 医学的に原因を特定できない高齢者の胸腹部症状に対して大柴胡湯を中心とした処方が奏効した2例を報告する。1例目は81歳の男性。胸部圧迫感が出現し,頻度が増加したため入院して精査を行ったが明らかな異常を認めなかった。便秘,胸脇苦満を目標に大柴胡湯を投与した。3ヵ月後には胸部圧迫感が消失し,気鬱スコアやSDS も改善した。2例目は83歳の男性。多発外傷後より,心窩部痛,胸腹部圧迫感,腹部膨満感が約2年間持続していた。心窩部痛と便秘,胸脇苦満を目標に大柴胡湯を投与したところ徐々に心窩部痛,胸腹部圧迫感の症状が改善し不定期受診の回数も減少した。大柴胡湯は抑うつ症状を伴い,医学的に原因が特定できない胸腹部症状に対して考慮すべき方剤である。
ばね指は,手のA1輪状靭帯の部位に狭窄を生じ円滑な指屈伸動作が阻害されることにより発症し,西洋医学的には消炎鎮痛剤,ステロイド剤注入,手術等で治療される。今回温経湯が奏効したばね指の3症例を報告する。症例1は71歳女性。腹部膨満感に対して漢方治療中に右第3指のばね指を発症し,口唇乾燥と手足のほてりを認めた。 症例2は56歳女性。手指の多関節痛に対して漢方治療中に左第4指のばね指を発症し,手のほてりを認めた。症例3は71歳女性。慢性腎不全で加療中に,左第1指のばね指を発症した。手のほてりや口唇乾燥を認めなかったが,皮膚の枯燥感を認めた。温経湯は,手掌煩熱や口唇乾燥を使用目標としており,補血・駆瘀血作用や抗炎症作用,滋潤作用を有する生薬で構成されている。温経湯のこれらの作用が,ばね指改善に寄与した可能性がある。
症例は68歳女性,当科受診の1年前に環状肉芽腫症を発症し,トラニラストで改善傾向にあった。しかし肝障害が出現し継続困難となり,皮疹が悪化したため漢方治療を試みることとなった。のぼせ,舌,皮疹の性状から熱候が示唆され,黄連解毒湯を開始し,改善傾向にあったが,入院3日目から治癒が横ばいになった。裏寒の存在を疑い,麻黄附子細辛湯の併用を開始したところ,開始5日目から急速に肉芽の縮小を認めた。臨床的に改善を認めたことより,陽証と陰証の併存があったと考える。
症例は56歳男性。12年前より下痢・心窩部不快感・腹部膨満感を認め,過敏性腸症候群の診断で多施設にて西洋医学的加療を受けたが,改善を認めず,5年前より漢方治療を開始した。しかし漢方方剤を服用しても数日で腹部膨満感が増悪するため服薬困難であり,処方変更の度に同様のことを繰り返したため,入院加療の方針となった。腹が張る・ガスがたまるなどの持続的な腹部膨満感を認めると共に,日に数回,急激に腹が張るような,発作的な腹部膨満感が出現するため,発作的な腹部膨満感を奔豚気ととらえ苓桂甘棗湯を処方開始した。入院後速やかに発作的腹部膨満感は消失した。入院12日目に臍周囲に他覚的冷感を認め,苓桂甘棗湯の類方で寒を散じ痛みを止める良姜の加わった良枳湯に転方したところ,持続的な腹部膨満感も消失し,入院24日目に退院した。良枳湯は原典では塊痛右にあるものを治すとあるが,必ずしも右季肋部の症状を伴わずとも,奔豚気に冷えを伴う症例に良枳湯が有効である可能性が考えられる。
