This first prospective observational study evaluates the impact of extended versus segmental colorectal surgery on risk metachronous cancer (CRC) in patients with Lynch syndrome, analyzing data from Prospective Syndrome Database version 5. Extended resection significantly reduced CRC path_MLH1, path_MSH2, and path_MSH6 carriers compared to resection.
