Background— “Therapeutic angiogenesis” seeks to improve perfusion by the growth of new blood vessels. The Regional Angiogenesis with Vascular Endothelial factor (RAVE) trial is first major randomized study adenoviral vascular endothelial (VEGF) gene transfer for treatment peripheral artery disease (PAD). Methods and Results— This phase 2, double-blind, placebo-controlled was designed test efficacy safety intramuscular delivery AdVEGF121, a replication-deficient adenovirus encoding...

Delivery and tracking of endomyocardial stem cells are limited by the inability to image transplanted noninvasively in beating heart. We hypothesized that mesenchymal (MSCs) could be labeled with a iron fluorophore particle (IFP) provide MRI contrast vivo assess immediate long-term localization.MSCs were isolated from swine. Short-term incubation MSCs IFP resulted dose-dependent efficient labeling. Labeled remained viable for multiple passages retained vitro proliferation differentiation...

A prospective analysis of 421 patients undergoing coronary artery bypass graft (CABG) surgery as the sole cardiac procedure was carried out to assess frequency central nervous system (CNS) complications. In all, 451 variables were assessed in each patient. Stroke occurred 5.2% but severe only 2%. Prolonged encephalopathy 11.6% usually resolved before discharge. No statistically significant pre- or intraoperative risk for CNS complications found; specifically, age pump times excess two hours...

Background Commercial low-field-strength MRI systems are generally not equipped with state-of-the-art hardware, and suitable for demanding imaging techniques. An system was developed that combines low field strength (0.55 T) high-performance technology. Purpose To evaluate applications of a system, specifically MRI-guided cardiovascular catheterizations metallic devices, diagnostic in high-susceptibility regions, efficient image acquisition strategies. Materials Methods A commercial 1.5-T...

Transcatheter aortic valve replacement (TAVR) has become a widely accepted procedure for treating patients with symptomatic stenosis. While transfemoral access remains the primary route due to its lower complication rates and favorable outcomes, subset of have anatomical or clinical factors precluding this approach. For these patients, alternative routes such as transaxillary, transcarotid, transcaval provide viable options. This expert consensus statement aims comprehensive review case...

The local environment of delivered mesenchymal stem cells (MSCs) may affect their ultimate phenotype. MR fluoroscopy has the potential to guide intramyocardial MSC injection desirable targets, such as border between infarcted and normal tissue. We tested ability (1) identify infarcts, (2) navigate catheters preselected (3) inject safely even into fresh (4) confirm success immediately.A 1.5-T MRI scanner was customized for interventional use, with rapid imaging, independent color highlighting...

Inflammation contributes to atherosclerotic plaque remodeling, enlargement and rupture. Non-invasive imaging of coronary artery inflammation could help target therapy 'vulnerable' atheromata, but is limited because small tissue mass arterial motion. Local radiopharmaceutical may overcome some these limitations. We used a positron-sensitive fiberoptic probe, which can distinguish positron emissions from annihilation photons, identify diseased healthy endothelium in an model. New Zealand White...

Direct intramyocardial injection may permit local delivery of protein and gene therapy agents for myocardial coronary artery disease. Little is known about the immediate fate materials administered via percutaneous endomyocardial catheters or surgical epicardial injection. In this study, we use a novel method to evaluate acute retention injected directly into myocardium, compare with endocardial postmortem delivery, influence injectate volume on retention. Fifteen 40-50 kg pigs underwent...

We tested the feasibility of targeted left ventricular (LV) mural injection using real-time MRI (rtMRI).A 1.5T scanner was customized with a fast reconstruction engine, transfemoral guiding catheter-receiver coil (GCC), MRI-compatible needle, and tableside consoles. Commercial imaging software to facilitate catheter navigation visualization injections at 4 completely refreshed frames per second. The aorta traversed cavity entered under direct rtMRI guidance. Pigs underwent multiple dilute...
