ナムセンスとは, 患者が体性感覚刺激に対して偶然を超えた高い確率で正しく反応することができるにもかかわらず, 同じ刺激を意識的に知覚することができない現象である。従来のナムセンスは, 触覚と触点定位との乖離という視点で議論されてきた。近年我々は, 触れた物体にナムセンスが生じる症例を報告した。患者は左頭頂葉梗塞後, 体性感覚刺激の大部分を認識することができなかったが, 形, 肌理, 物品に関する刺激を偶然を超えた高い確率で正しく選択することができた。失行報告と本例のナムセンスについて, 特徴と考えられる病態機序を解説した。

頭頂弁蓋部を含む病巣に伴い体性感覚障害を呈した, 2 症例を報告した。2 例とも基本的体性感覚が重度に障害されていたが, 皮質性体性感覚のうち立体幾何図形, 手触り素材, 日用物品の同定についてチャンスレベル以上に正しく反応し, 体性感覚の階層的なモデルに合致しなかった。また, 日常生活で不便を感じないほどに巧緻動作が行え, 体性感覚障害が運動制御を障害するという考え方に反していた。うち 1 例では, 皮質性の体性感覚の実感が全くないのにもかかわらず, これらの課題に正しく反応していることがうかがわれた。これらの現象の機序を, 触覚情報処理の つの流れや第二体性感覚皮質の機能に関する理論に基づいて考察した。
