A 78-year-old man, who had previously undergone coronary artery bypass graft surgery, was admitted to our department for treatment of a distal aortic arch aneurysm. total replacement with patent left internal thoracic (LITA) successfully performed without cardiac ischemic or neurological complications. To reduce the risk of stroke during open surgery for treatment descending thoracic or thoracoabdominal aortic diseases, we attempted to insert a proximal elephant trunk stabilize atherosclerosis at site anastomosis.
