The PulseRider (Cerenovus, Johnson & Medical Devices, New Brunswick, NJ, USA) is a neck reconstruction device that used for the treatment of unruptured wide-necked bifurcation aneurysms. Herein, we describe case 51-year-old male patient with basilar apex aneurysm who was treated but had post-procedural brainstem infarctions caused by one proximal markers covering origin perforator. 脳底動脈の血豆状動脈瘤(blood blister-like aneurysm: BBA)に対して,ステントアシストコイル塞栓術を行い,治癒できた症例を経験したため報告する.症例は53歳男性で,頭痛にて救急搬送され,頭部CTでくも膜下出血と診断された.出血源が明らかでなく,フォローアップのDSAで上位脳底動脈側壁の膨隆を徐々に認め,出血源と診断し,第25病日にステントアシストコイル塞栓術を同部位に施行した.治療直後からblebの造影はなくなり,半年後のフォローアップDSAでは,母血管壁にwhite collar signも認め,治癒と判断した.本症例では,ステントアシストコイル塞栓術を施行し,コイル塞栓による確実な止血とネックブリッジステントの整流効果によって,根治が得られたと考えた.
