Renal angiomyolipomas are a frequent manifestation of tuberous sclerosis and sporadic lymphangioleiomyomatosis (LAM). These disorders associated with mutations TSC1 or TSC2 that lead to overactivation mTOR complex 1 (mTORC1), suggesting an opportunity for targeted therapy by using mTORC1 inhibitors. This study investigated the efficacy safety inhibitor sirolimus treatment renal in patients these disorders. In this multicenter phase 2 nonrandomized open label trial, 16 LAM angiomyolipoma(s)...

Percutaneous transthoracic lung biopsies are commonly performed for the investigation of masses. We describe current practice and complication rates in UK. A postal questionnaire was sent to all centres British Thoracic Society directory. 157 replies (61% response rate) were received, providing data on 5444 biopsies. Mean number per annum 30.5 centre; 8% did not perform biopsies, 36% <25 annum, 34% <50, 16% <100 6% >100. Consultant radiologists 91% Written consent obtained at centres. The...

Background
Laboratory studies suggest that the clotting cascade is activated in fibrotic lungs. Since humans vary their tendency to clot due a variety of inherited or acquired defects, we investigated whether prothrombotic state increases chance developing idiopathic pulmonary fibrosis (IPF) and/or worsens prognosis IPF.
Methods
We recruited 211 incident cases IPF and 256 age- sex-matched general population controls collected data on medical history, medication, smoking...

Lymphangioleiomyomatosis is a rare disease occurring almost exclusively in women. Diagnosis often requires surgical biopsy and the clinical course varies between patients with no predictors of progression. We evaluated recent diagnostic guidelines, features serum biomarkers as prognostic tools.Serum vascular endothelial growth factor-D (VEGF-D), angiotensin converting enzyme (ACE), matrix metalloproteinases (MMP) -2 -9, phenotype, thoracic abdominal computerised tomography, lung function...

Background Rare cystic lung diseases are increasingly recognised due the wider application of CT scanning making disease management a growing part respiratory care. Cystic tend to have extrapulmonary features that can both be diagnostic but also require surveillance and treatment in their own right. As some these now specific treatments, precise diagnosis is crucial. While Langerhans cell histiocytosis, Birt-Hogg-Dubé syndrome, lymphoid interstitial pneumonia lymphangioleiomyomatosis...

Metastatic pulmonary calcification occurs in association with several disease processes and has a highly variable prognosis. In certain patients, it may prove fatal, therefore prompt recognition of radiological signs is helpful for optimising patient management. We describe the progressive changes seen on CT 59-year-old man who had undergone renal transplant subsequently developed metastatic calcification. The merits different imaging techniques available possible treatment options are reviewed.

Solitary pulmonary nodule (SPN) is a common clinical problem, and the history vital in assessing pre-test probability of malignancy. Previous relevant imaging should be accessed. CT scanning for an SPN to defined protocol, so that any follow-up studies are comparable. Positron emission tomograpy depreotide both offer non-invasive characterization, which, with improved image registration, could use even nodules than 10 mm diameter.
