症例は64歳の男性で,上部内視鏡検査で胃体上部小彎前壁に主座を持ち,食道胃接合部まで浸潤する3型腫瘍を認めた.腹部造影CTとPET-CTで胃噴門~体部小彎にかけて漿膜浸潤を疑う原発巣を認め,S5,S7に多発肝転移と,腹腔動脈根部から大動脈分岐部付近まで及ぶ傍大動脈リンパ節転移を認めた.生検検体による病理組織学的検査でHER2 scoreは3+であり,capecitabine+cisplatin+trastuzumab(初回~術前8 mg/kg)療法を開始した.8コース目以降はcisplatin抜きで計25コース終了後,肝転移巣とリンパ節転移巣の著明な縮小を確認し,脾摘と大網切除を伴う開腹胃全摘,D2を施行した.病理組織学的検査では,pT2,N0,ly1,v0,pStage IBであった.術後trastuzumab単独投与(6 mg/kg)を1か月毎に26コース継続し,初診時から7年半経過した現在,無再発生存中である.多発肝転移と傍大動脈リンパ節転移を伴う切除不能HER2陽性胃癌に対し集学的治療で長期生存した1例を経験したので報告する.
