横隔膜ヘルニアは経皮的ラジオ波焼灼術(RFA)後遅発合併症の一つであり,嵌頓などで重症化する報告が散見される.症例は60歳台男性.主訴は便秘,腹痛および発熱.前医にて多発肝細胞癌(HCC)に対し,RFAを計20回受けた.来院時CTにて右横隔膜の一部欠損および胸腔内への結腸の脱出を認め,腸管拡張および腸液貯留を認めたため,横隔膜ヘルニア嵌頓の診断にて緊急手術施行となった.ヘルニア門は3.5×2.5cmであり,横行結腸と大網が脱出し,ヘルニア門に癒着していた.癒着剝離およびヘルニア門の一部切開を行い,用手的に横行結腸と大網を還納した.腸管の血流不全は認めなかった.ヘルニア門は縫合閉鎖の後,Composite Meshで補強した.RFA施行後横隔膜ヘルニアは嵌頓症状をきたすことがあり,予後不良症例を認めるため,機を逸することがない様に手術的治療を施行する必要があると考えられる.
症例は73歳,男性.血便を主訴に下部消化管内視鏡検査施行したところ,下部直腸に2型病変を認めた.また,上部消化管内視鏡検査にて胃体下部大彎に0-Ⅱc病変と小彎に0-Ⅱb病変を認めた.下部直腸癌はT4a,N1,M0と診断,下部直腸癌が予後を規定すると判断し,術前化学放射線療療法を施行した後に直腸癌切除する方針とした.術前化学放射線療法後の直腸病変は瘢痕化しており内視鏡的にほぼComplete Responseであった.術前化学放射線療法終了2カ月後に手術を施行した.手術は腹腔鏡下に同時切除を予定した.腹腔鏡下幽門側胃切除術を先行させ,続いて腹腔鏡下腹会陰式直腸切断術を施行した.胃の再建はBillroth-Ⅰ法(デルタ吻合)で行った.通常,腹部に胃の切除標本を取り出すための小開腹創を置くが,腹会陰式直腸切断術の会陰創から切除標本を摘出した.本症例は胃癌・直腸癌を同時切除することで小開腹創を置くことなく腹腔鏡下に手術を施行しえた.
