愛知県西三河地区の市中病院で2008年から2010年までの3年間に分離同定され,抗菌薬感受性検査を実施した緑膿菌1343株を対象に,抗菌薬感受性結果の推移を分析した.その結果,カルバペネム系抗菌薬に対する耐性率はIPM/CSで6.4% vs 3.3% (2008年対2010年のデータを対比,以下同様),MEPMで2.7% 1.4%と低下傾向を示した.同時にCPFXの耐性率も13.4% 5.5%と低下した.アミノグリコシド系抗菌薬であるAMKの耐性率は0.2% 0.6%と低率を維持した.対照的にβ–ラクタム系抗菌薬に対する耐性率は上昇傾向を示した.この推移は同期間におけるカルバペネム系抗菌薬の投与量の減少や相対的な第3,第4世代のセフェム系抗菌薬の使用比の増加と連動していた.緑膿菌の耐性率の改善の背景には,カルバペネム系抗菌薬の適正使用に向けた取り組みのみならず,感染制御活動の成果やDPC導入に伴う入院期間の短縮など複合的な要因が挙げられる.多剤耐性緑膿菌の検出は他院からの持ち込みの1例のみであった.今後も抗菌薬感受性サーベイランスに基づく抗菌薬の適正使用に向けた質の高い活動の継続が必要である.
