A definitive diagnosis of gastroesophageal reflux disease (GERD) depends on endoscopic and/or pH-study criteria. However, high resolution manometry (HRM) can identify factors predicting GERD, such as ineffective esophageal motility (IEM), esophago-gastric junction contractile integral (EGJ-CI), evaluating esophagogastric (EGJ) type and straight leg raise (SLR) maneuver response. We aimed to build externally validate a manometric score (Milan Score) stratify the risk severity in patients...

Abstract Background Literature suggests colonic resection and primary anastomosis (RPA) instead of Hartmann’s procedure (HP) for the treatment left-sided emergencies. We aim to evaluate surgical options globally used treat patients with acute emergencies factors that leading choice treatment, comparing HP RPA. Methods This is a prospective, international, multicenter, observational study registered on ClinicalTrials.gov. A total 1215 who required surgery were included from 204 centers during...

High-resolution manometry (HRM) allows assessment of esophagogastric junction (EGJ) disruption. While type 3 EGJ predicts definitive gastroesophageal reflux disease (GERD), 2 is less clearly implicated in GERD pathogenesis. This study aimed to characterize physiologic findings determine if the HRM-based Milan Score can define within EGJ.

ABSTRACT Introduction According to Lyon 2.0, laryngopharyngeal symptoms (LPS) should undergo upfront pathophysiologic tests. The novel Milan score integrates esophagogastric junction (EGJ) morphology, ineffective esophageal motility, EGJ‐contractile integral (EGJ‐CI), and straight leg raise (SLR) response. It has been demonstrated predict abnormal AET. aim of this study was assess the value in predicting GERD these patients. Methods We prospectively enrolled patients with suspected who...

Objective: To explore the manometric characteristics of patients with severe GERD. assess role HRM features in adjunctive diagnostic Lyon 2.0 criteria and to create a GERD severity prediction model. Summary Background Data: is complex disorder challenges, as symptoms may not reflect disease severity. High-resolution manometry Milan Score numerically quantify degree anti-reflux barrier disruption symptoms. Methods: An international prospective database undergoing pH-impedance (MII-pH) for...

The straight leg raise (SLR) maneuver during high-resolution manometry (HRM) can assess esophagogastric junction (EGJ) barrier function by measuring changes in intraesophageal pressure (IEP) when intra-abdominal is increased. We aimed to determine whether increased esophageal SLR predicts pathologic acid exposure time (AET).Adult patients with persistent gastroesophageal reflux disease (GERD) symptoms undergoing HRM and pH-impedance or wireless pH study off proton pump inhibitor were...

Esophageal cancer (EC) remains one of the most common and aggressive diseases worldwide. This review discusses some debates in modern management disease. Endoscopic procedures for early (T1a-b) are now embedded routine care challenge will be to more accurately select patients endoscopic resection with or without adjuvant therapy. Perioperative multimodal therapies associated improved survival compared surgery alone locally advanced esophageal cancer. However, there is no global consensus on...

Mucinous adenocarcinoma associated with an anal fistula is a rare oncologic entity which may pose diagnostic and therapeutic challenges for Surgeons Medical Oncologists. Few reported cases without definite guidelines exist. It represents 2-3% of all gastrointestinal malignancies arises from chronic fistulas, ischiorectal or perianal abscesses. Achalasia is a primary oesophageal motility disorder. Although aetiology remains mainly unknown, genetic risk variant, rs28688207 in HLA-DQB1, showed strong achalasia association suggesting involvement of immune-mediated processes the pathogenesis. High-resolution manometry recognises three types achalasia. The aim our study was to perform first genotype-phenotype analysis investigating frequency across high-resolution subtypes.
