To compare the clinical and cost effectiveness of total hip arthroplasty with resurfacing in patients severe arthritis hip.Single centre, two arm, parallel group, assessor blinded, randomised controlled trial 1:1 treatment allocation. One large teaching hospital United Kingdom.126 older than 18 years joint, suitable for hip. Patients were excluded if they considered to be unable adhere procedures or complete questionnaires.Total (replacement entire femoral head neck); articular surface only,...
The orientation of the acetabular component in metal-on-metal hip resurfacing arthroplasty affects wear rate and hence failure. This study aimed to establish if interpretation pelvic radiographs with TraumaCad software can provide a reliable alternative CT measuring inclination version. was used measure on AP pelvis 14 painful resurfacings. Four orthopaedic surgeons performed each measurement twice. These were compared measurements taken from reformats. correlation between calculated, as...
Postoperative urinary retention requiring urethral catheterisation increases the risk of joint sepsis following arthroplasty. Spinal anaesthesia with opiate administration is used widely in lower limb We sought to establish whether choice agent had any effect on incidence postoperative and therefore sepsis.A total 445 consecutive patients who underwent primary elective arthroplasty were reviewed retrospectively. Patients general femoral nerve block (GA+FNB), spinal intrathecal fentanyl...
Background: Computer navigation and patient-specific instrumentation have been in use over the past 2 decades for total knee replacement (TKR). However, their effects on implant survival patient-reported outcomes remain under debate. We aimed to investigate influence survival, of Oxford Knee Score (OKS) health-related quality life (EQ-5D-3L), intraoperative complications, postoperative mortality compared with conventional instrumentation, across a real-world population. Methods: This...
We used Laser Doppler flowmetry to measure the effect on blood flow femoral head/neck junction of two surgical approaches during resurfacing arthroplasty. studied 24 hips undergoing arthroplasty for osteoarthritis. Of these, 12 had a posterior approach and trochanteric flip approach. A probe was placed under radiological control in superolateral part junction. The flux measured at stages operation compared with initial flux. In both groups main fall occurred exposure capsulotomy hip joint....
To compare the medium-term clinical effectiveness of total hip arthroplasty and resurfacing arthroplasty.Single centre, two-arm, parallel group, assessor blinded, randomised controlled trial with 1:1 treatment allocation.A large teaching hospital in England.122 patients older than 18 years severe arthritis joint, suitable for hip. Patients were excluded if they considered to be unable adhere procedures or complete questionnaires.Total (replacement entire femoral head neck); articular surface...
There is no consensus on the optimal form of venous thromboembolic prophylaxis treatment in hip and knee arthroplasty patients, or safety complication profile available chemical modalities. In this study we aimed to measure return theatre rate for any cause related wound complications patients undergoing total replacement replacement, compare these rates between oral Rivaroxaban 10mg OD subcutaneous Enoxaparin 40mg our department. were a 387 included study; 227 group 1, who received OD, 160...
Background: Large-diameter (≥36-mm) metal-on-metal (MoM) total hip replacements have been shown to fail at an unacceptably high rate. Globally, the DePuy Pinnacle prosthesis was most widely used device of this type. There is evidence suggest that one main reasons for poor clinical performance large-diameter MoM prostheses metal debris released from head-stem taper junction—i.e., junction failure. The aim study investigate variation in as-manufactured finish female surface and determine its...
The current generation of metal-on-metal hip resurfacing designs has largely been characterized by cemented femoral fixation using a cementless cup. We present the clinical results 135 entirely uncemented procedures. primary outcome measures were revision for any cause and Oxford score at latest follow up. average length up was 2.9 years. mean 18.4 no patient required either component during study period. Uncemented may be comparable to with cement in resurfacing.
Abstract Aims Debate continues as to the optimal orientation of acetabular component in total hip arthroplasty (THA) and how reliably achieve this. The primary objective this study was compare functional CT-based planning patient-specific instruments with conventional THA using 2D templating. Methods A pragmatic single-center, patient-assessor blinded, randomized control trial patients undergoing performed. 54 (aged 18–70) were recruited either Corin Optimized Positioning System (OPS) or...
We performed a radiographic review of consecutive series 87 resurfacing arthroplasties the hip, between 2004 and 2006. There were 54 cemented femoral components 33 uncemented components. All procedures by same surgeon using approach. was no difference groups with regards to gender, age, pre-operative diagnosis average diameter The primary outcome measured neck-prosthesis ratio on standard anterior-posterior pelvis radiographs taken post-operatively at minimum follow up two years. immediate...
Harlequin ichthyosis (HI) is a rare autosomal recessive skin disorder. No orthopaedic procedure has previously been described on patient with HI. We report the case of 17-year-old HI who presented bilateral juvenile idiopathic arthritis hips underwent total hip replacements. Our standard operative and postoperative regime was followed no complications occurred. One year after second our had Oxford score 43 very satisfied result.
The primary aim of this study was to analyse our painful metal-on-metal (MoM) hip resurfacing revisions with evidence 'adverse reactions metal debris' (ARMD). In series 35 the median whole blood Cobalt levels were 58 nmols/l (range 12-1407 nmols/l), and Chromium 73 2-353 nmols/l). Thirty-four patients had abnormal imaging on Ultrasound scanning (USS). mean histological Campbell grading ARMD 4, ranged from 0-9. Oxford Hip Score (OHS) increased 19 pre-revision 4-46) 33 post-revision surgery...
We compared 47 patients with groin pain following hip resurfacing to a matched control group. Functional scores and plain radiographs were assessed along measurement of whole blood cobalt chromium by inductively coupled mass spectrometry. Symptomatic underwent ultrasound scan the affected hip. Mean functional outcomes poor in those good Groin was associated valgus stem positioning lower neck:head ratio (relatively narrow neck) (p=0.03, p=0.04 respectively). classified into two groups:...
Aim Predicting success of a Debridement, Antibiotics and Implant Retention (DAIR) procedure for Periprosthetic Joint Infection (PJI) remains challenge. A failed DAIR might adversely affect the outcome any future revision surgery PJI. Hence, ability to identify optimise factors predictive would help target appropriate patient cohort avoid unnecessary patients where is unlikely eradicate infection. Method retrospective review our prospective Bone Group database was performed all who underwent...

We report a retrospective review of femoral head/neck ratios on post-operative and two year follow-up radiographs following hip resurfacing arthroplasty. The patients were in matched groups, having had surgery through posterior approach or via Ganz trochanteric flip. There was no significant difference neck narrowing at follow up between the surgical approaches. However, we found both groups by time radiograph.
