The coronavirus disease 2019 (COVID-19) first reported in Wuhan, China December is a global pandemic that threatening the health and wellbeing of people worldwide. To date there have been more than 274 million cases 5.3 deaths. Omicron variant documented City Tshwane, Gauteng Province, South Africa on 9 November 2021 led to exponential increases sharp rise hospital admissions. clinical profile patients admitted at large Tshwane compared with previous waves.466 COVID-19 admissions since 14...
Background Lipoid proteinosis (LiP) is a rare autosomal recessive disorder characterized by hoarse voice, warty skin infiltration and scarring. Mutations within the extracellular matrix protein 1 (ECM1) gene cause LiP. Since early 1970s it has been recognized that South Africa one of largest groups LiP patients worldwide, suggesting probable founder effect. As present with considerable clinical variability, this group offers unique opportunity for genotype–phenotype correlation. Objectives...
Abstract Background Ralstonia species are Gram-negative bacilli of low virulence. These organisms capable causing healthcare associated infections through contaminated solutions. In this study, we aimed to determine the source mannitolilytica bacteraemia in affected patients a haemodialysis unit. Methods Our laboratory noted an increase cases caused by mannitililytica between May and June 2016. All underwent at unit academic hospital. The reverse osmosis filter water system was found be...
Screening for traditional risk factors of cardiovascular disease is well known in primary healthcare (PHC) settings. However, other through newer tools (such as bioelectrical impedance analysis [BIA]) could also be predictors increased (CVR). Body composition estimates (body fat percentage, body water lean mass) by BIA and its association to CVR have been studied with variable results.
Peritoneal dialysis (PD) peritonitis is a feared complication of PD, with significant sequelae for the patient. The cause PD largely due to single organism (≥75% cases) and rarely multiple organisms. In this pilot study, we investigated 25 cases 16S ribosomal RNA (rRNA) next-generation sequencing (NGS) techniques.Total concordance between culture NGS was noted. In addition, technique highly sensitive, identifying 33 different bacteria (including nonculturable bacterium), compared 13 bacterial...
HIV infection causes immune dysregulation affecting T-cell and monocyte function, which may alter coronavirus disease 2019 (COVID-19) pathophysiology. We investigated the associations among clinical phenotypes, laboratory biomarkers, hospitalisation outcomes in a cohort of people hospitalised with COVID-19 high prevalence area.We conducted prospective observational study Tshwane, South Africa. Respiratory severity was quantified using respiratory oxygenation score. Analysed biomarkers...
In South Africa, novel fungal pathogens have emerged as an important cause of disease in HIV-infected individuals. The clinical presentation is characteristically prolonged, insidious and the diagnosis elusive, often being made post mortem. We report individual with a systemic mycosis due to Aureobasidium pullulans, ubiquitous dematiaceous mould which has not been described
Introduction: Bacillus species are often considered as contaminants when cultured from clinical samples. cereus may be a pathogen in certain circumstances and is known to cause musculoskeletal infections. This report aims educate clinicians microbiology laboratories on B. infections heighten awareness that should not always dismissed contaminants. Case presentation: We the case of patient who presented tertiary hospital Pretoria, South Africa, November 2018 with septic arthritis underlying...

Nephronophthisis (NPHP) is a genetically heterogeneous ciliopathy characterized by corticomedullary cyst formation and fibrosis leading to end-stage kidney disease, often associated with extra-renal findings. This study explores the genetic basis of NPHP in South African family affected its juvenile form.
Background: Ralstonia species are environmental organisms prevalent in different types of water supplies, including hospital water. may cause serious infections the immune compromised host. This study describes interventions employed during an outbreak mannitolilytica bacteraemia among patients undergoing haemodialyis at Steve Biko Academic Hospital (Pretoria, South Africa) nephrology department between May 2016 and July 2016. Methods & Materials: Sixteen with R.mannitololytica haemodialysis...
The Apolipoprotein L1 (APOL1) gene confers an increased risk for developing nondiabetic kidney disease in people of African descent. Specifically, when more than one APOL1 allele (either the G1 or G2 allele) is present, HIV associated nephropathy, focal segmental glomerulosclerosis hypertension nephropathy raised multiple fold. Recent evidence however has questioned role variants Black South patients with hypertensive nephropathy.
Acute coronary syndromes due to artery vasospasm occur rarely and are difficult diagnose in the acute setting. We present a patient with hypocalcaemia-induced vasospasm, which resulted an ST-elevation myocardial infarction. This was reversible upon administering intravenous calcium, no long-term cardiac consequences for our patient.Hypocalcaemia is common clinical problem that usually clinically quiescent, but setting can dramatic signs symptomsHypocalcaemia resulting syndrome rapidly...
Acute kidney injury (AKI) is a common complication of COVID-19 infection, with 17% hospitalized patients developing AKI. While the majority AKI episodes appear minor (±50% stage 1), presence increases mortality risk 15 fold or more. Surprisingly, incidence related to lower than all-cause (22%, estimated before pandemic). This contrasted against rate, where pre-COVID-19 was at 24%, while 39%, and increasing 53% for requiring continuous renal replacement therapy ICU admission.
Introduction: Acute kidney injury (AKI) in hospitalized patients infected with COVID-19 is associated an elevated mortality rate compared to non-infected (39% versus 24%). This despite a lower prevalence of AKI COVID-19-infected (17 vs 22%). The reasons are multifactorial and have been well documented developed countries, whereas developing countries there scant data. Methods: study aimed document the South African who required dialysis for AKI. Exclusion criteria included any chronic...
Abstract Background : HIV is moderate risk factor for developing severe COVID-19 and associated with increased of mortality. infection causes immune dysregulation characterised by progressive lymphopenia, chronic activation, immunological senescence, T cell exhaustion. These changes are partly reversed effective antiretroviral therapy (ART), which reduces morbidity mortality in people living (PWH). We investigated the associations among clinical phenotypes, laboratory biomarkers,...
Systemic Lupus Erythematosus (SLE) is characterised by an hyperactive immune system, with loss of self-tolerance core to the pathophysiology. The nonsynonymous variant rs1143679 A in integrin-αM (ITGAM) gene strongly associated development SLE phenotype non-African populations, while no association has been found renal disease any populations. allele diminishes ITGAM function, disallowing effective phagocytosis, resultant inefficient removal antigen/immune complexes. This may lead chronic...
Peritoneal dialysis (PD) is a well-established form of renal replacement therapy, but relatively underutilized and decreasing in use worldwide. The concept PD was introduced over century ago by Wegner 1877, as long-term therapy (RRT) began the 1960's. largest hurdle for RRT with PD, then now, peritoneal access peritonitis. Peritonitis, serious complication frequently arising from touch contamination, can result considerable mortality morbidity.
