Abstract Appropriate patient selection and minimizing complications are critical for successful spinal cord stimulation (SCS) therapy in managing intractable pain. We thus reviewed electronic medical records of 707 consecutive cases patients who received SCS the Cleveland Clinic from 2000 to 2005 with an emphasis on indications complications. was used treat complex regional pain syndrome (CRPS) (345 cases), failed back surgery (235 peripheral vascular disease (20 visceral chest, abdomen,...

Many patients with diabetic peripheral neuropathy experience chronic pain and inadequate relief despite best available medical treatments. To determine whether 10-kHz spinal cord stimulation (SCS) improves outcomes for refractory painful (PDN).The prospective, multicenter, open-label SENZA-PDN randomized clinical trial compared conventional management (CMM) SCS plus CMM. Participants PDN 1 year or more to gabapentinoids at least other analgesic class, lower limb intensity of 5 cm on a 10-cm...

Knee osteoarthritis is a painful and sometimes debilitating disease that often affects patients for years. Current treatments include short-lasting repetitive nonsurgical options, followed by surgical intervention appropriate candidates. Cooled radiofrequency ablation (CRFA) minimally invasive procedure the treatment of pain related to knee osteoarthritis. This trial compared efficacy safety CRFA with those single hyaluronic acid (HA) injection. Two hundred sixty subjects was inadequately...

ObjectivesTotal knee arthroplasty (TKA) is an effective surgery for end-stage osteoarthritis, but chronic postoperative pain and reduced function affect up to 20% of patients who undergo such surgery. There are limited treatment options, percutaneous peripheral nerve stimulation (PNS) a promising nonopioid option chronic, persistent pain. The objective the present study was evaluate effect 60-day PNS in multicenter, randomized, double-blind, placebo-controlled trial treating after...

Spinal cord stimulation (SCS) has been challenged by the lack of neurophysiologic data to guide therapy optimization. Current SCS programming trial-and-error results in suboptimal and variable therapeutic effects. A novel system with a physiologic closed-loop feedback mechanism using evoked-compound action potentials enables optimization neural dose consistently accurately activating spinal fibers. We aimed identify metrics their ranges that resulted clinically meaningful treatment responses.

To evaluate high-frequency (10-kHz) spinal cord stimulation (SCS) treatment in refractory painful diabetic neuropathy.A prospective, multicenter randomized controlled trial was conducted between Aug 28, 2017 and March 16, 2021, comparing conventional medical management (CMM) with 10-kHz SCS+CMM. The participants had hemoglobin A1c level of less than or equal to 10% pain greater 5 10 cm on visual analog scale, neuropathy symptoms 12 months more gabapentinoids at least 1 other analgesic class....

Safe and effective non-surgical treatments are an important part of the knee osteoarthritis (OA) treatment algorithm. Cooled radiofrequency ablation (CRFA) hyaluronic acid (HA) injections two commonly used modalities to manage symptoms associated with OA.A prospective 1:1 randomized study was conducted in 177 patients comparing CRFA HA injection follow-ups at 1, 3, 6 12 months. subjects unsatisfactory outcomes 6-months were allowed crossover receive CRFA. Background: The SENZA-PDN study evaluated high-frequency 10-kHz spinal cord stimulation (SCS) for the treatment of painful diabetic neuropathy (PDN). Over 24 months, SCS provided sustained pain relief and improved health-related quality life. This report presents additional outcomes from study, focusing on diabetes-related life outcomes. Methods: randomized 216 participants with refractory PDN to receive either conventional medical management (CMM) or plus CMM (10-kHz + CMM), allowing...

Chronic postsurgical pain (CPSP) can be caused by peripheral nerve injury (PNI) resulting from surgical procedures and has a significant neuropathic component. This prospective, single-arm study was conducted to document the effectiveness of 10-kHz spinal cord stimulation (10-kHz SCS) as treatment for patients with CPSP.Subjects CPSP who were refractory conventional medical interventions reported scores ≥5 cm on 10-cm VAS underwent trial stimulations lasting up 14 days. Abstract Objective To evaluate the longer‐term efficacy of 10 kHz spinal cord stimulation (SCS) in managing painful diabetic neuropathy (PDN) a routine clinical setting after transition from 24‐month SENZA‐PDN study. Methods We contacted 142 participants who completed 24 months postimplantation follow‐up former randomized controlled trial (SENZA‐PDN). Of these, 57 consented and responded to this post‐study survey. Outcomes assessed included pain relief, health‐related quality life (HRQoL)...

Intrathecal drug delivery (IDD) system with implantable pumps has been used to treat cancer-related pain as well noncancer-related chronic pain. Opioids, including morphine and hydromorphone, are the most commonly intrathecal (IT) agents. Although technology, techniques, knowledge of IDD have improved, dose escalation occurs relatively rapidly in noncancer pain.Retrospective chart review pump patients, implanted for a minimum 2 years, was designed investigate possible existing predictors...
