A 59-year-old man, who had lived in Sapporo and eaten raw salmon trout until 1988, admitted our hospital because of the positive fecal occult blood test. 症例は57歳女性。嘔気にて近医で上部消化管内視鏡検査を受け,胃角部小彎側に巨大潰瘍を指摘された。良性潰瘍と判断されランソプラゾールの投与を受け,約2カ月後の再検査では胃角部小彎側の潰瘍は縮小していたが,胃体部の壁伸展不良と皺襞の肥厚があり,生検で低分化型腺癌+印環細胞癌と診断された。その後当院へ入院し胃全摘・脾合併切除を行った。癌組織の進展範囲は胃のほぼ全域であったが,内視鏡検査で指摘された潰瘍は瘢痕化していた。スキルス胃癌に大きな癌巣内潰瘍を合併し,経過中にその潰瘍が瘢痕化に至った稀な症例であった。今回のように当初の内視鏡所見では良性潰瘍と思われたものが実際には癌巣内潰瘍である場合もあり,生検等を加えて診断することが肝要である。
