Inflammatory myofibroblastic tumor (以下, IMT) は肺を好発部位とする腫瘤であるが, 下部消化管に発生することは非常にまれである. 今回, 急速に増大し, 悪性腫瘤との鑑別が困難であった虫垂原発のIMTを経験したので報告する. 症例は58歳の男性で, アルコール性肝硬変にて当院内科通院中, 右下腹部の腫瘤を触知, その後腫瘍の増大傾向が認められ入院精査となった. CTにて盲腸から背側に虫垂と一塊になった造影効果を伴う腫瘤を認めたが, 下部消化管内視鏡検査にて盲腸が下方から壁外性に圧排されているのみで腫瘍性病変は認めなかった.また, 注腸造影X線検査にて上行結腸から盲腸にかけ多発する憩室を認め, 盲腸は変形していた. 以上より, 盲腸周囲の炎症性変化を疑うが, 増大傾向があり悪性腫瘍も否定できず, 腹腔鏡下回盲部切除術を施行, 病理組織学的検査にてIMTと診断された.
