Rationale: Uncontrolled inflammatory innate response and impaired adaptive immune are associated with clinical severity in patients coronavirus disease (COVID-19).Objectives: To compare the immunopathology of COVID-19 acute respiratory distress syndrome (ARDS) that non-COVID-19 ARDS, to identify biomarkers mortality ARDS.Methods: Prospective observational monocenter study. Immunocompetent diagnosed RT-PCR-confirmed severe 2 (SARS-CoV-2) infection ARDS admitted between March 8 30, 2020, were...
The benefit of high-dose dexamethasone and oxygenation strategies vs standard care for patients with severe acute hypoxemic respiratory failure (AHRF) caused by COVID-19 pneumonia is debated. To assess the compared dexamethasone, to high-flow nasal oxygen (HFNo2) or continuous positive airway pressure (CPAP) support (o2SC).This multicenter, placebo-controlled randomized clinical trial was conducted in 19 intensive units (ICUs) France from April 2020 January 2021. Eligible were consecutive...
Abstract Editor’s Perspective What We Already Know about This Topic Article Tells Us That Is New Background Neutrophil extracellular traps have been associated with tissue damage. Whether these are involved in the pathogenesis of human acute respiratory distress syndrome (ARDS) and could be a potential therapeutic target is unknown. The authors quantified bronchoalveolar blood neutrophil patients pneumonia-related ARDS assessed their relationship ventilator-free days. Methods Immunocompetent...
Patient hospitalized for coronavirus disease 2019 (COVID-19) pulmonary infection can have sequelae such as impaired exercise capacity. We aimed to determine the frequency of long-term capacity limitation in survivors severe COVID-19 and factors associated with this limitation. Patients were enrolled 3 months after hospital discharge COVulnerability, a prospective cohort. They underwent cardiopulmonary testing, function test, echocardiography, skeletal muscle mass evaluation. Among 105...
Given the high risk of thrombosis and anticoagulation-related bleeding in patients with hypoxemic COVID-19 pneumonia, identifying lowest effective dose anticoagulation therapy for these is imperative. To determine whether therapeutic (TA) or high-dose prophylactic (HD-PA) decreases mortality and/or disease duration compared standard-dose (SD-PA), TA outperforms HD-PA; to compare net clinical outcomes among 3 strategies. The ANTICOVID randomized open-label trial included pneumonia requiring...
Abstract Background Biomarkers of disease severity might help individualizing the management patients with acute respiratory distress syndrome (ARDS). Whether alveolar compartmentalization biomarkers has a clinical significance in pneumonia-related ARDS is unknown. This study aimed at assessing interrelation ARDS/sepsis and blood compartments explored their association outcomes. Methods Immunocompetent admitted between 2014 2018 were included prospective monocentric study. Bronchoalveolar...
Enterocytozoon bieneusi microsporidiosis is an emerging disease in immunocompromised patients. We report 2 cases of this allogeneic hematopoietic stem cell transplant patients successfully treated with fumagillin. Thrombocytopenia occurred but without major adverse events. Modifications immunosuppression could be avoided when E. rapidly identified and fumagillin therapy started promptly.
We report a patient with an unusual initial metabolic presentation of imported human rabies who became symptomatic within 2 weeks returning from Mali to France. This is the single case identified in France past 11 years and first viral RNA bronchial secretions.
Abstract Background The respective benefits of high and low doses dexamethasone (DXM) in patients with severe acute respiratory syndrome coronavirus 2 (SARS-Cov2) failure (ARF) are controversial, two large triple-blind RCTs reaching very important difference the effect-size. In COVIDICUS trial, no evidence additional benefit high-dose (DXM20) was found. We aimed to explore whether some specific patient phenotypes could from DXM20 compared standard care 6 mg dose DXM (DXMSoC). Methods...
Biomarkers of disease severity might help with individualizing the management patients acute respiratory distress syndrome (ARDS). During sepsis, a sustained decreased expression antigen-presenting molecule human leucocyte antigen-DR (HLA-DR) on circulating monocytes is used as surrogate marker immune failure. This study aimed at assessing whether HLA-DR alveolar in setting severe lung infection associated their functional alterations. BAL fluid and blood from immunocompetent...
A 57-year-old woman hospitalized for a COVID-19 (coronavirus disease-2019)–related refractory acute respiratory distress syndrome developed few days later anteroseptal ST-segment elevation with systolic dysfunction. Coronary angiography was performed the patient in prone (face down) position, owing to necessity maintain reasonable oxygen saturation during examination. (Level of Difficulty: Intermediate.)
