目的: 以HER2荧光原位杂交(FISH)检测作为参照,使用免疫组织化学(IHC)法比较2种抗体对胆囊腺癌HER2蛋白检测效能。 方法: 建立一组包含108例胆囊浸润性腺癌的高产组织芯片,用2种抗HER2抗体(克隆号4B5和A0485)在其相对应平台检测HER2蛋白表达,FISH方法检测HER2基因扩增。用ROC曲线计算IHC灵敏度、特异度及准确度,Kappa系数分析一致性,McNemar检验比较各组间检出率差异。 结果: HER2基因扩增病例为23例(21.3%)。4B5抗体的表达结果(3+、2+、0/1+)为12、13、83例。IHC 3+检出HER2基因扩增的灵敏度和特异度分别为52.2%和100%。A0485抗体的表达结果(3+、2+、0/1+)为22、45、41例。IHC 3+检出HER2基因扩增的灵敏度和特异度分别为65.2%和68.2%。两组结果差异有统计学意义(P<0.01)。以IHC 2/3+作为阳性,4B5抗体检出HER2扩增的灵敏度和特异度分别为78.3%和72.0%,A0485为100%和34.3%,结果差异有统计学意义(P<0.01)。4B5抗体IHC...
