甲状腺眼症の多くはバセドウ病に合併し,外眼筋肥大などにより眼球突出等の症状をきたし,重篤な際には圧迫性視神経症により失明に至ることもある。視神経症の治療はステロイドパルス療法が第一選択で,治療効果を認めない際には眼窩減圧術が必要となる。今回我々は,甲状腺眼症による視神経症例に対して鼻内内視鏡下での眼窩減圧術を行った2症例を経験したので報告する。症例1は53歳女性で,症例2は51歳女性である。両者ともバセドウ病による甲状腺眼症であり,症例2は2回目の放射線ヨード治療を契機に視神経症を発症した。症例1は右眼,症例2は両眼の急激な視力低下をきたし,ステロイドパルス療法で視力の改善が無いため眼窩減圧術目的に当科に紹介となった。手術は鼻内内視鏡下に両側の眼窩内側壁と下壁内側の骨壁を除去し,骨膜を切開して眼窩内容物を篩骨洞内に逸脱させることで減圧した。両症例とも術後の視力は著明に回復し,およそ一年後に斜視手術を行うことで術後に悪化した複視も消失した。

今回われわれは,緊急気管切開を要した血管浮腫の1例を経験したので報告する。症例は,62歳男性である。頸部,舌の腫脹を主訴に当科を受診した。舌および喉頭に高度な浮腫を認めた。血管浮腫の診断で緊急気管切開を施行したが,術中に呼吸苦の増悪を認め,呼吸停止した。呼吸停止直後輪状甲状膜切開を行い,気道を確保した。アンギオテンシン変換酵素阻害薬,アンギオテンシンII受容体拮抗薬を内服しており血管浮腫の原因薬として疑い休薬し,その後浮腫の再発を認めていない。アンギオテンシン変換酵素阻害薬の副作用として血管浮腫が知られており,稀ではあるが重篤な経過をたどることがあり注意を要する。
