Despite international differences in cultural perspectives on end-of-life issues, little is known of the care for dying general wards acute hospitals Asia.We performed a retrospective medical chart review all 683 adult patients who died without intensive unit (ICU) admission our Singaporean hospital 2007. We first evaluated prevalence do-not-resuscitate (DNR) orders and or against life-sustaining therapies; second, if such were discussed with and/or family members; third, actual treatments...

Abstract Background Durvalumab consolidation is associated with improved survival following concurrent chemoradiotherapy (CCRT) in patients stage III non‐small cell lung cancer (NSCLC). Given the heterogeneity of NSCLC patients, this study we evaluated efficacy and safety durvalumab real‐world setting. Method Unresectable were retrospectively studied: one cohort received CCRT, another had CCRT‐durvalumab. Primary endpoints progression‐free (PFS) overall (OS), secondary relapse rate safety....

Abstract Partial anomalous pulmonary venous return (PAPVR) is a rare congenital heart condition which often asymptomatic and hence remains undiagnosed, but could occasionally be detected on thoracic imaging as an incidental finding. For cases of newly diagnosed hypertension with concurrent PAPVRs, the diagnostic workup subsequent management are more complicated, requiring thorough evaluation secondary causes hypertension, assessing relative PAPVR shunt contribution to hypertension. We herein...

Immune-related adverse events (IrAEs) of immune checkpoint inhibitors (ICIs) can be serious and unpredictable. We examine the incidence rate risk factors for IrAEs in an Asian cohort nonsmall cell lung cancer (NSCLC) patients treated with immunotherapy. Between June 2014 August 2020, we retrospectively analysed NSCLC anti-PD-1 or anti-PD-L1 at National University Cancer Institute, Singapore. A Poisson regression model was used to estimate effect on any grade IrAEs. One hundred forty-one were...

Primary resistance to immune checkpoint inhibitors (ICI) is observed in routine clinical practice. We sought determine factors predictive of primary ICI monotherapy, defined by the Society for Immunotherapy Cancer (SITC) as progression within 6 months treatment with patients receiving at least weeks advanced non-small-cell lung cancer (NSCLC).Patients stage IV NSCLC treated single-agent two tertiary hospitals Singapore were included. A multivariate logistic regression model was utilised...

Endobronchial ultrasound (EBUS) combined with trans-esophageal endoscopic bronchoscope guided fine need aspirate (EUS-B FNA) of mediastinal lymph nodes is an established procedure for diagnosis. The main barrier to a EBUS EUS-B FNA approach availability trained and accredited pulmonologist who can perform safely confidently. To address this gap, we undertook training program experienced bronchoscopists train, learn, incorporate into their procedural practice. Thirty-two patients were...

Abstract Background and objective Simulation enhances a physician's competency in procedural skills by accelerating ascent of the learning curve. Training programmes are moving away from Halstedian model ‘see one, do teach one’, also referred as medical apprenticeship. We aimed to determine if 3‐month structured bronchoscopy curriculum that incorporated simulator training could improve among pulmonary medicine trainees. Methods prospectively recruited trainees hospitals with accredited...

Convex probe endobronchial ultrasound transbronchial needle aspirations (CP-EBUS-TBNAs) and radial lung biopsies (RP-EBUS-TBLBs) can be performed under moderate sedation or general anesthesia. Moderate is more convenient, however patient discomfort may result in inadequate tissue sampling. General anesthesia ensures better cooperation but requires logistics also carries risks. We aim to describe the diagnostic yield safety of CP-EBUS-TBNAs RP-EBUS-TBLBs when at our center. All patients who...

Spontaneous regression (SR) of lung cancer is especially rare. We report a case an early endobronchial squamous cell carcinoma that exhibited complete radiologic and pathologic SR soon after diagnosis. The patient was monitored radiologically bronchoscopically at regular intervals to date, the biopsy samples have been negative. Remarkably, has maintained 6 years diagnosis, without local recurrence or distant metastases. Many hypotheses abound explain how achieved despite lack treatment.
