千葉大学医学部皮膚科で加療した外陰部Paget病39例の治療結果をまとめ, 治療方針の検討を試みた。自験例は, 男性29例, 女性10例で, 男女共に60歳代が最も多かった。原発巣の深達度は, (1)表在性10例, (2)面皰癌11例, (3)真皮乳頭層までの浸潤癌7例, (4)真皮網状層より深い浸潤癌11例であった。深達度(4)の11例中9例に鼠径リンパ節転移が認められ, 後腹膜リンパ節の郭清も施行した2例では大動脈下部リンパ節まで転移が認められた。リンパ節転移陽性例は原発巣に硬結や腫瘤, 潰瘍形成を伴っていた。原発巣の大きさ, 発疹に気づいてから受診までの期間, 臨床的なリンパ節腫脹は, 必ずしもリンパ節転移の目安にはならなかった。鼠径リンパ節郭清を施行する目安は, 原発巣の浸潤が真皮網状層におよぶ場合であり, 郭清したリンパ節の中に, 構築の破壊された進行したリンパ節転移が1個でもあれば, 骨盤内リンパ節郭清を追加した方がよい。しかし, それが数個におよぶ場合は, 化学療法や放射線療法のような他の治療法を選択した方が, 生命予後のために良いのではないかと考えられた。
