内視鏡下に根治切除を行った鼻副鼻腔良性腫瘍23例, 悪性腫瘍4例について検討を加えた. 内視鏡下良性腫瘍切除の適応は鼻腔, 上顎洞内側壁, 篩骨洞, 蝶形骨洞前壁に存在し, 前頭洞, 上顎洞前壁, 蝶形骨洞, 眼窩, 翼口蓋窩へ腫瘍が大きく進展していない症例とした. 悪性腫瘍切除の適応は鼻腔, 鼻腔に面した副鼻腔壁に存在し, en bloc切除可能な小腫瘍で, 組織学的に低悪性度の症例とした. 良性腫瘍の組織型は内反性乳頭腫10例, 血管腫9例, 若年性血管線維腫2例, 多形腺腫1例, 線維腫1例であり, 悪性腫瘍では嗅神経芽細胞腫, 腺房細胞癌, 扁平上皮癌, 軟骨様脊索腫であった. 再発性内反性乳頭腫2例, 軟骨様脊索腫1例を除き, bloc切除が可能であった. 良性腫瘍では術後平均観察期間は21ヵ月で再発例はなかった. 悪性腫瘍では軟骨様脊索腫症例で術後早期に再発した. 内視鏡下にサルベージ手術を行い, 術後陽子線治療を行った. サルベージ手術後3年5ヵ月間再発を認めない. 術前塞栓術, 内視鏡下蝶口蓋動脈結紮術, レーザーを中心とした焼灼機器を使用することにより良性,...
