症例は58歳, 男性. 1991年12月に慢性糸球体腎炎による末期腎不全のため血液透析導入となり, 以後維持透析を受けていた. 2005年11月頃から好酸球増多症が出現し, 2006年8月からダイアライザーと透析回路内に残血を認めるようになった. ヘパリン増量や透析膜の変更を行ったが残血量に変化はなく, 10月頃から透析開始直後に一過性の胸部不快感と血圧低下が出現するようになった. アスピリン服用により残血とショックは消失したが, 掻痒感が出現したためアスピリンを中止したところ, 再び透析時の残血とショックが出現した. 透析後の著明な血小板減少に加え, ヘパリン・PF4複合体抗体 (HIT抗体) が陽性を呈したことから, II型のヘパリン起因性血小板減少症 (HIT) と診断した. 抗凝固薬をメシル酸ナファモスタットに変更した後は, 透析中の凝血, 血小板減少およびショックは軽快し, 好酸球増多症も軽減した. 透析患者でのHITはほとんどが導入時に発症するが, 本例では透析導入後15年目に初発し, さらに好酸球増多症を伴う透析中のショック症状を認めた稀有な1例であった.

58歳男性。1年前, 右肩に硬結が出現し, 半年前より急激に増大。初診時, 右肩に手拳大の弾性硬, 表面びらんを伴うパトカーランプ様円柱状紅色腫瘤(6.5×7×5cm)を認めた。生検組織で紡錐形細胞がstoriform patternを呈し, CD34陽性であったことなどから隆起性皮膚線維肉腫 (DFSP) と診断した。入院後, 腫瘍辺縁より2cm離し, 下床は僧帽筋の一部を含めて全摘し, 分層植皮術を施行した。全摘組織の病理はDFSPの所見に加え, 一部に紡錐形細胞の高密度増殖巣がみられた。これらの領域ではinterlacing bundleやherringbone 細胞分裂像も多く, CD34の染色性も低下していた。このような所見は従来DFSP with fibrosarcomatous changeの名称で報告されてきたものと同一のものと考えた。これまでの本邦報告17例と海外報告例を検討し, DFSPとの違いを考察した。
