症例は70歳，男性．下腿浮腫，下痢を主訴に当院受診した．貧血・低アルブミン血症を認め，精査の結果，上行結腸肝彎曲部に全周性2型腫瘍と十二指腸下行部への瘻孔形成を認めた．CT・MRIで，膵浸潤も認めた．結腸十二指腸瘻を伴う上行結腸癌と診断し，結腸右半切除術・膵頭十二指腸切除術（以下PD）を施行した．病理診断はpT4b（膵，十二指腸），muc＞tub1，pN0，pStage IIであった．術後補助化学療法としてFOLFOX4を6カ月間行った．術後16カ月間再発なく経過している．大腸癌は他臓器浸潤を認めても完全切除が望ましい．結腸十二指腸浸潤症例では，侵襲が大きくなるが，膵頭十二指腸切除を行うことで予後の改善が期待できると考えられた．

未分化多形肉腫は，組織学的に特定の分化傾向を示さず，多形性を呈する腫瘍として悪性線維性組織球腫が再分類されたものである．成人軟部肉腫の中で最も頻度が高いが，消化管原発例は極めてまれで，十二指腸原発のものはこれまでに6例が報告されているのみである．今回，我々は十二指腸原発未分化多形肉腫の1例を経験したので報告する．症例は69歳の男性で，心窩部痛精査の腹部造影CT・超音波内視鏡検査で十二指腸筋層外から膵頭部実質に浸潤する25 mm大の腫瘤を指摘された．超音波内視鏡下針生検（endoscopic ultrasonography fine needle aspiration biopsy；EUS-FNA）にて類円形・紡錐形異型細胞の充実性増殖を認め，肉腫や分化度の低い癌が疑われた．遠隔転移を認めず，膵頭十二指腸切除術を施行した．病理診断は十二指腸未分化多形肉腫であった．術後6か月で頸部と坐骨近傍の軟部組織，左副腎に転移を来したため，術後再発に対する化学療法としてdoxorubicin投与を開始し，現在術後10か月経過している．
