大腸癌の異時性他臓器重複癌の中で,小腸癌との組み合わせは極めて稀である.今回,直腸癌治癒切除術後の原発性小腸癌の1例を経験したので報告する.症例は70歳,女性,平成11年4月,直腸癌にて腹会陰式直腸切断術を施行した(高分化腺癌,a1,n0,stageII).平成12年末より鉄欠乏性貧血を認め,上部と下部消化管内視鏡検査を施行したが異常を認めず,その後の経過観察中も貧血を繰り返すため小腸造影を施行した.小腸に腫瘤像を認め,小腸腫瘍も考慮し平成13年3月,小腸部分切除術を施行した.中分化腺癌,seの原発性小腸癌であった.直腸癌術後の異時性小腸癌は,われわれが検索した限り極めて稀であった.癌の手術の既往のある患者において,持続する貧血に対し,小腸腫瘍も考慮し小腸造影を含め精査が必要であると考えられた.
