今回われわれは,上顎歯肉に初発症状を認めた肺原発上顎歯肉転移癌を経験したためその治療経過の概要に文献的考察を加えて報告する。患者は80歳男性,初診時右側上顎歯肉に,直径18mm弾性軟の腫瘤を認めた。生検の結果,大細胞癌と診断された。歯肉原発の可能性が低く,全身精査を実施したところ,左側上肺野,胃,脳に腫瘍性病変を認め,肺原発多発転移癌の診断を得た。上顎歯肉転移癌は生検後急速に増大したため,S-1を併用した同時化学放射線療法を実施し,照射量合計38.4Gyで終了した。これらの放射線治療によって上顎歯肉転移癌の縮小を認めた。治療後,脳梗塞を発症し死の転帰となったが,死の直前まで腫瘍の増大もなく,経口摂取の継続が可能であった。根治治療の適応がない肺原発歯肉転移癌に対する,姑息的同時化学放射線療法は,患者のQOL維持に有効な治療法の1つであると思われた。

Salivary duct carcinoma of the submandibular gland presenting a diagnostic challenge: A case report

症例は初診時年齢59歳男性，右側口底扁平上皮癌（T2N0M0）の診断にて，下顎辺縁切除を伴う口底部分切除術を施行した。術後40か月目に右側頸部リンパ節に後発転移を認め，FDG-PETで同リンパ節とともに両側肺尖部に異常集積があり，肺転移を疑った。癌化学療法にて頸部と肺病巣ともに一度は縮小とSUVmaxの低下を認めたが，その後頸部リンパ節のみ再増大し，肺病巣はSUVmax低下を示した。頸部郭清術を施行した後，肺病変は非結核性抗酸菌症であることが判明した。初回口底癌治療から7年6か月，頸部郭清術から3年7か月経過し，再発・転移を認めず，経過良好である。肺非結核性抗酸菌症は近年増加傾向にあり，肺転移や肺原発腫瘍に類似した画像を呈するため注意が必要である。
