子育て女性医師を支援する法律,制度,施設は充実してきたが,それらを利用する女性麻酔科医の背景と取り巻く環境は実に多様で,一概に当てはめることは困難である.仕事と家庭のバランスの考え方は一人ひとり異なり,それぞれのスタイルで人生を謳歌していけばよいが,医師を志したときの自分の気持ち,周囲の応援を一たび思い起こせば,そう簡単に仕事を諦めることもないのではないかと思う.女性麻酔科医の第三世代として,自分のこれまでを振り返りながら,子育て女性医師を活用することで生じるメリット,子育て女性医師がうまくやっていくコツについて述べ,第四世代の先生方につなげたい.

ブピバカイン6mgにフェンタニル0.02mgを併用した脊椎麻酔とブピバカイン12.5~15mg単独の脊椎麻酔との循環動態の変化を後ろ向き研究で比較した.フェンタニル併用群13例では高齢者にもかかわらず収縮期血圧の低下が少なく,昇圧薬の使用例は2例であった.ブピバカイン単独使用群は20例中10例で昇圧薬が必要となった.また,フェンタニル併用群はより少ない輸液量でも低血圧を起こしにくいことが明らかになった.ブピバカインにフェンタニルを併用する脊椎麻酔は,高齢者などの昇圧薬の使用や輸液管理に,注意が必要な患者に適した麻酔法であることが示唆された.
