症例は57歳の女性で,腹部刺創で当院に救急搬送となり,精査の結果,外傷性腹腔動脈損傷,肝損傷,膵損傷の診断で,緊急開腹術の方針とした.術中所見では肝損傷,膵損傷に加え腹腔動脈の完全断裂を認め,腹腔動脈の結紮により止血を得た.総肝動脈が上腸間膜動脈から分岐するAdachi分類VI型の血管走行異常を有していたため,術後も肝血流は温存され,術後34日目に軽快退院した.腹腔動脈が断裂している症例においては結紮での止血により肝臓や胃の血流障害が危惧されるが,その血管走行異常によっては腹腔動脈の血流が途絶しても臓器の血流が温存される症例が存在する.今回,Adachi VI型の血管走行異常を伴う外傷性腹腔動脈損傷の1救命例を経験したのでこれを報告する.

盲腸脂肪腫による腸重積に対し腹腔鏡補助下回盲部切除を施行し,良好な結果が得られたので若干の文献的考察を含めて報告する.症例は73歳男性.下腹部痛を主訴に救急受診した.一旦症状は軽快したが,3日後に再度症状が増悪したため入院のうえ精査した.以前から指摘されていた,Bauhin弁上に存在するφ50mm大の脂肪腫による腸重積が腹痛の原因であると診断し,待機的に腹腔鏡補助下回盲部切除を施行した.術後経過は良好であった.腹腔鏡下手術は侵襲が少ないため,良性疾患である脂肪腫に対して非常に良い適応であると考えられた.
