The increase in prevalence of liver disease globally will lead to a substantial incremental burden on intensive care requirements. While transplantation offers potential life-saving intervention, not all patients are eligible due limitations such as organ availability, resource constraints, ongoing sepsis or multiple failures. Consequently, the focus critical with advanced and decompensated cirrhosis turns liver-centric protocols, mitigate high mortality patients.

Background and Objectives: Sepsis still represents a syndrome with high mortality. A timely sepsis diagnosis an early intervention are crucial for the disease outcomes. Sepsis-associated acute kidney injury (SA-AKI) is highly prevalent but often diagnosed late. We aimed to investigate whether serum interleukin-6 (IL-6) leukocyte cell population data (CPD) could be adequate biomarkers prediction of survival SA-AKI development. Materials Methods: conducted prospective observational study in...

Noni (Morinda citrifolia) juice is a popular herbal dietary supplement globally used for preventive or therapeutic purposes in variety of ailments, claiming to exhibit hepatoprotective properties as well. Herein we present the case 38-year-old woman who developed acute liver injury associated with noni consumption on long-term (9 months) anticonvulsant therapy. Clinical presentation and biopsy were consistent severe, predominantly hepatocellular type injury. Both agents stopped...

West Nile virus (WNV) has become one of the new challenges for transplant programs. In addition to transmission by mosquito bite, interhuman is possible through blood products or organ transplantation. Majority WNV infections present as asymptomatic mild febrile illness, with less than 1% infected developing neuroinvasive disease. Many studies report naturally acquired donor-derived in solid-organ recipients, mainly kidney, but also liver, heart, lungs and pancreas. Given much higher risk...

The synchronous or metachronous coexistence of gastrointestinal stromal tumors (GISTs) with solid and hematologic neoplasms has been addressed in a non-transplant population. However, the association primary hepatic leukemias is uncommon. Scarce data exist considering GISTs other transplant population where long-term immunosuppression carries additional burden de novo malignancy. We present case posttransplant GIST acute biphenotypic leukemia patient transplanted for intrahepatic...

Liver transplantation has now become the standard of care for individuals with end-stage liver disease or acute failure.1 This advancement led to a rising need hepatologists skilled in transplant medicine, leading development hepatology education. Trainees are required master clinical management advanced diseases and address ethical psychosocial aspects while also caring recipients managing recurrence and/or post-transplant complications.1 Consequently, becoming hepatologist requires...

Insulin resistance is associated with increased risk of death and liver transplantation in the cirrhotic population, independent disease aetiology. However, factors accounting for insulin context cirrhosis are incompletely understood. This study aimed to investigate association between adiponectin leptin patients assess influence severity on metabolic status. cross-sectional included 126 non-diabetic transplant candidates. Background and objectives: Endothelial dysfunction has been proposed to be an underlying mechanism of the pronounced cardiovascular morbidity in end-stage liver disease (ESLD), but clinical evidence is still limited. In this study, we investigated association circulating levels asymmetric dimethylarginine (ADMA) nitric oxide (NO) with estimated risk patients ESLD awaiting transplantation. Materials Methods: ADMA NO were measured sera 160 adult patients. The severity hepatic was assessed by...

Liver transplantation has become standard practice for treating end-stage liver disease. The success of the procedure relies on effective immunosuppressive medications to control host's immune response. Despite liver's inherent capacity foster tolerance, early post-transplant period is marked by significant reactivity. To ensure favorable outcomes, it imperative identify and manage various rejection types, encompassing T-cell-mediated, antibody-mediated, chronic rejection. However, approach...

(1) Background: Allogeneic hematopoietic stem cell transplantation (allo-HSCT) is a lifesaving treatment but carries high infection risk. Diagnosing infections remains challenging due to the limited accuracy of standard biomarkers. (2) Methods: This single-center study aimed evaluate presepsin (PSP) and YKL-40 as biomarkers in febrile patients during allo-HSCT pre-engraftment phase. Biomarker levels were prospectively measured 61 episodes from 54 at admission, representing baseline then Day...
