We introduce a new technique of thoracoscopic esophagectomy with the patient in prone position. 自治医科大学の五十嵐によって現在の形態の硬膜外内視鏡 (エピドラスコピー) がわが国に紹介されて約10年が経過した. われわれも現在まで試行錯誤を繰り返し, さらに症例を経験することにより治療成績の向上を目指して自分たちなりの手順や手技を作り上げ, ある程度満足のいく結果が得られるようになってきた. 当施設におけるエピドラスコピーの手技, 術後管理など初期の頃と変遷したものを述べてみたい.
