【目的】腎不全の右頚部内頚動脈高度狭窄の患者に,ほぼ造影剤を使用することなく,頚動脈ステント留置術(carotid artery stenting; CAS)を施行した1 例を報告する.【方法】大腿動脈に9Fr OPTIMO を挿入,頚椎MRI- 頚動脈MRA fusion image を参考にして総頚動脈に留置,エコーガイド下にPercuSurge GuardWire を外頚動脈に留置した.総頚動脈および外頚動脈のバルーンを拡張し順行性血流を遮断し,少量の希釈造影剤で撮影後,直ちに吸引回収した.Intra-vascular ultrasoundgraphy(IVUS)で狭窄病変長,内頚動脈および総頚動脈径を計測し,前拡張,エコーガイド下にステント留置,後拡張を行った.IVUS で拡張を確認して手技を終了した.【結果】術後経過問題なく,5 日後に退院した.Follow up の頚動脈エコー上ステント開存は良好であった.【結論】造影剤使用不可能症例でも,頚動脈エコー,頚椎MRI- image,IVUS を利用してCAS を安全に施行でき,本法は有用と考えられた.
