セツキシマブは,EGFRを標的としたモノクローナル抗体で,現在,頭頸部癌における唯一の分子標的薬である。米国のNCCNガイドラインでは,局所進行頭頸部扁平上皮癌治療の選択肢として,セツキシマブ併用放射線療法が記載されている。当院では,現在まで頭頸部扁平上皮癌11例に適用した。内訳は,中咽頭癌6例,下咽頭癌2例,喉頭癌3例。Stage II4例,III3例,IVA4例。男性10例,女性1例で,年齢は49~81歳(中央値63歳),PSは全例0であった。ほぼ全例で,Grade3の放射線性皮膚炎・口腔咽頭粘膜炎が生じたが,Grade4の有害事象は生じず,全例で治療完遂できた。治療終了12週間後の評価は,全体でCR9例,PR2例であり,Stage III/IVではCR5例,PR2例であった。今後,症例数を積み重ね,長期成績も含めた検討が必要であると考えられた。

2015年米国甲状腺学会(ATA)ガイドライン改訂に伴い,当院における甲状腺髄様癌(MTC)治療との比較を行った。2000年~2016年の間に当院で診断・治療されたMTCは5例(男性2名,女性3名)で,3例が術前細胞診でMTCを疑われた。RET遺伝子検査は1例術前,3例術後に施行し,散発性3例,MEN2A1例であった(1例拒否)。MTCを疑う場合,ATAガイドラインでは全摘術を推奨している。一方術後にMTCと判明した散発性症例に関して,補完全摘は必ずしも推奨されない。当院では散発性2例に非全摘術を施行し,追加治療は施行していない。今後の方針として,術前遺伝学的検査の徹底をすること,治療は甲状腺全摘術を原則(症例により非全摘も考慮)とすることを確認した。
