A bstract : We studied survival and disease complications in 1,146 patients with thalassemia major, born from January 1, 1960 to December 31, 1987. At last follow‐up, March 1997, probability of age 20 years was 89% 25 82% for the 1970‐1974. Patients who died had a serum ferritin level, measured year before death, significantly higher than those survived. Diabetes present 5.4% patients; heart failure 6.4%; arrhythmias 5.0%, thrombosis 1.1%, hypothyroidism 11.6%, HIV infection 1.8%....
Background It has been repeatedly reported that female patients with thalassemia major survive longer than males and the difference is due to a lower rate of cardiac disease in females.Design Methods We compared iron load as measured by T2* magnetic resonance imaging 776 (370 males) examined at National Research Council part an Italian cooperative study. also established normal left ventricular ejection fraction values for our population. Results The prevalence was higher females (105 versus...
Abstract We report data on survival and complications for a longitudinal cohort of 709 transfusion‐dependent β ‐thalassemia major patients (51.1% males) born between 1970 1997 followed through 2020 at seven centers in Italy. Overall probability 30 years was 83.6% (95%CI: 78.5–89.1) the oldest birth (1970–1974) compared with 93.3% 88.6–98.3) youngest (1985–1997) ( p = 0.073). Females showed better than males 0.022). There were total 93 deaths median age 23.2 most frequent disease‐related...
Hepatocellular carcinoma (HCC) frequently complicates hepatic cirrhosis secondary to viral infection or iron overload. Therefore, patients affected by thalassaemia syndromes have a theoretically high risk of developing the tumour. We collected data on attending Italian centres for treatment thalassaemia. Twenty-two cases HCC were identified; 15 male. At diagnosis, mean age was 45 +/- 11 years and serum ferritin 1764 1448 microg/l. Eighty-six percent had been infected hepatitis C virus....
BACKGROUND: Hypertransfusion with a baseline hemoglobin of 10 to 12 g per dL is still considered by many be the mainstay conservative therapy for beta‐thalassemia major. However, this regimen frequently associated manifestations transfusion iron overload, despite regular chelation subcutaneous desferoxamine. STUDY DESIGN AND METHODS: To verify whether target pretransfusion level between 9 and can allow significant reduction in blood consumption, while effectively suppressing erythropoiesis,...
Abstract A segmental, multislice, multi‐echo T 2 * MRI approach could be useful in heart iron‐overloaded patients to account for heterogeneous iron distribution, demonstrated by histological studies. However, segmental assessment can affected the presence of geometrical and susceptibility artefacts, which act on different segments ways. The aim this study was assess value distribution left ventricle develop a correction procedure compensate artefactual variations analysis. performed four...
Summary The risk of developing hepatocellular carcinoma ( HCC ) in patients with thalassaemia is increased by transfusion‐transmitted infections and haemosiderosis. All Italian Thalassaemia Centres use an ad hoc form to report all diagnoses the Registry. Since our last report, 2002, up December 2012, 62 new cases were identified, 52% whom affected major TM 45% intermedia TI ). Two had sickle‐thalassaemia ST incidence tumour increasing, possibly because longer survival consequent exposure...
Objectives: Iron chelation treatment (ICT) in β-thalassemia major (β-TM) patients undergoing blood transfusions can cause low satisfaction, compliance, with possible negative consequences on success, patients' wellbeing, and costs. The purpose was to estimate the societal burden attributable β-TM terms of direct indirect costs, health-related quality-of-life (HRQoL), satisfaction compliance ICT ICT.Research design methods: naturalistic, multicenter, longitudinal...
A 16‐year‐old boy with congenital hepatic fibrosis, on whom a portocaval anastomosis was performed because of bleeding esophageal varices, developed lip cyanosis, exertional dyspnea, and clubbing the fingers 7 years postoperatively. This progressive syndrome caused by right‐to‐left shunt due to presence pulmonary arteriovenous fistulas. Increased erythrocyte sedimentation rate, hypergammaglobulinemia, histological features cholangitis were also present. It is proposed that this unusual...
Summary The age at which it is necessary to start Cardiovascular Magnetic Resonance ( CMR ) T2* screening in thalassaemia major TM still uncertain. To clarify this point, we evaluated the prevalence of myocardial iron overload MIO ), function and fibrosis by patients younger than 10 years. We retrospectively selected 35 enrolled Myocardial Iron Overload Thalassaemia network. was measured multislice multiecho technique. Biventricular parameters were cine images. detect fibrosis, late...
