目的:従来,全部鋳造冠の支台歯形成についてさまざまな教育方法や評価方法が試みられてきたが,動画教材を用いて教育効果を検討したものや支台歯の客観的評価に関する報告は数少ないように思われる.そこで,本研究は効果的な教材と妥当性のある支台歯の評価方法の開発を目的として行った.方法:術者の視界を録画することができるように位置を調整したカメラを用いて記録した,動画教材を用意した.10名の研修歯科医がこの動画教材を視聴する前と後に行った支台歯形成の結果を,面積計算を用いたテーパーに関する客観的手法と上級医の主観的評価によって比較することにより,本教材の効果を検証した.さらに客観的評価と主観的評価の一致率についても検討を行った.結果:動画視聴後にはすべての支台歯において主観的評価が上がっており,支台歯のテーパー,形態については動画視聴前後で有意差が認められた.客観的評価には有意差が認められなかったが,主観的評価と強い相関を示していることがわかった.結論:今回開発した動画教材は支台歯形成の教育に効果的であることがわかった.さらに,支台歯のテーパーを客観的に評価する際には面積計算が有用であること,...

症例の概要:患者は51歳の女性で下顎左側大臼歯部と上顎両側中切歯,上顎右側側切歯が欠損しており,明確なアンテリアガイダンス,咬合支持を失っていた.そこで,まず咬合支持の改善を優先的に行った後,プロビジョナルレストレーションを利用してアンテリアガイダンスを再構築した.考察:はじめに咬合支持の確立を行ったことにより,臼歯離開咬合を確実に付与することができた.結論:アンテリアガイダンスの修復が求められる症例においては,臼歯部での咬合支持を確立した後にプロビジョナルレストレーションを用いて適切なガイドを模索し,カスタムインサイザルテーブルによって最終補綴装置に反映させる方法が効果的である.
