要旨: 音楽幻聴は, 外部からの音刺激がないのに歌や旋律が自然に聞こえる現象であり, 耳鳴患者の中にも稀に存在する。2011年1月から2018年10月までに当科を受診した耳鳴患者のうち, 音楽幻聴を訴えた23例の臨床像を検討した。このうち11例に対し耳鳴について詳細に説明した上で補聴器による音響療法を行い, 治療効果を検討した。音楽幻聴症例は, 高齢者・女性に多く1例を除く全例で感音難聴を認めた。全例で病識が保たれており, 精神神経科疾患の合併を認めなかった。治療後に Tinnitus Handicap Inventory の合計値, 耳鳴の自覚的大きさ・苦痛の Visual Analogue Scale は有意に改善した。本検討から, 精神神経科疾患の合併がなく難聴が主病因の音楽幻聴に対し耳鳴の説明と補聴器による音響療法が有効な治療である可能性が示唆され, 耳鼻咽喉科医が中心となり診療に携わることが望ましいと考えられた。
唾液誤嚥により肺炎を繰り返すなどの高度嚥下障害に対する外科的治療として,気道と食道を分離する誤嚥防止手術が行われている。声門閉鎖術(鹿野式)をはじめ多くの誤嚥防止手術はカニューレフリーを目的として狭窄しにくい永久気管孔を造設するように工夫している。一方,術後人工呼吸器管理が必要な症例ではカフ付き気管カニューレの装着が必要となる。気管カニューレは気管切開孔への装着を目的に設計されているために永久気管孔と適合しないことが多い。誤嚥防止手術後のカニューレ不適合は気管切開術と比較して気管カニューレの脱落や気管腕頭動脈瘻などの重篤な合併症のリスクが上昇する。今回われわれは術後気管カニューレの装着を必要とする7例の患者に対して輪状軟骨を温存して声門閉鎖を行い,一般的な気管切開孔の形状を残す「輪状軟骨温存型声門閉鎖術」を行った。本術式における永久気管孔の曲率半径は声門閉鎖術(鹿野式)と比較したところ有意に小さく,気管カニューレの曲率半径に近似していた。われわれが本術式を施行した7例中5例に術後気管カニューレが装着され,術後経過は良好で重篤な合併症を認めていない。輪状軟骨温存型声門閉鎖術は気管カ...
要旨: 慢性耳鳴に対する治療として, 教育的カウンセリングや音響療法, 認知行動療法などが行われており, 特に難聴を伴う耳鳴に対しては補聴器による音響療法も普及してきている。今回, 難聴を伴う慢性耳鳴に対して宇都宮方式聴覚リハビリテーションで補聴器調整を行った症例95例を対象とし, 耳鳴改善効果について評価した。治療前の平均 THI 値は55±24点であったが, 治療開始3カ月後の平均 値は22±23点, 治療開始1年間の 最低値の平均は14±18点であり, 治療前と比較し有意に低下した。THI 値低下率は75%, 全症例の92%で 値が軽症化または20点以上の低下を認め, 難聴を伴う慢性耳鳴に対する補聴器による音響療法は有効な治療法であると考えられた。また両側耳鳴・片側耳鳴群では 値低下率に有意差は認めなかった。聴力左右差が20dBHL 以上の症例では, 20dBHL 未満の症例と比較して 値低下率が高く, 改善した症例の割合が多い傾向があった。
