症例は 50 歳,女性。左下腿の虫刺様皮疹に対し市販外用薬を塗布していたが,同部が潰瘍を伴う隆起性病変となったため,1 カ月後,当科を受診した。初診時,左下腿内側に径 3 cm の黄色痂皮が付着した隆起性潰瘍病変を認め,ヨードコート®軟膏外用を開始したが悪化したため,壊疽性膿皮症などを疑った。血液検査で血清クロール濃度(Cl)が 156 mEq/l(正常:98∼108 mEq/l)と異常高値であったが,4 年前から高 Cl 血症であったことが判明した。詳細な問診の結果,約 10 年間,慢性頭痛に対してブロモバレリル尿素含有の市販鎮痛薬(ナロン®顆粒)を内服していることが判明した。臭素摂取過量による偽性高 血症を呈したと考え,血中臭素濃度を測定したところ 500 mg/l(正常 5 mg/l 以下)と中毒域であった。皮疹辺縁部の皮膚生検では表皮は偽癌性増殖を呈し,真皮内には膿瘍形成を認め,臭素疹と確定診断した。 その後,ナロン®顆粒の内服を中止したところ速やかに は正常化し,皮疹も色素沈着となり軽快した。...
