Attitude towards preoperative biliary drainage for malignant obstructive jaundice have recently changed twice. This is due certain factors including complications of minimally invasive drainage, level bile duct block, cholangitis, need neoadjuvant chemotherapy, time to scheduled surgery, severe general condition patient, future liver remnant volume. We comprehensively searched PUBMED, MD Consult and National Library Medicine using the following keywords: “obstructive (OJ)”, “cellular...
To summarize the results of pancreaticoduodenectomy (PD) for pancreatic tumors and complications chronic pancreatitis regarding prediction prevention postoperative complications. There were 336 PD procedures between 2016 mid-2022 in two centers. We assessed factors influencing specific (postoperative pancreatitis, fistula, gastric stasis, arrosive bleeding). Several risk distinguished: baseline disease tumor size, CT-signs a «soft» gland, intraoperative assessment pancreas, number functioning...
To evaluate the immediate results of enucleation pancreatic neuroendocrine tumors (pNETs).The (pNETs) were analyzed in 95 patients between 2016 and 2021. Functioning (mean size 16.8 mm) found 70 patients, non-functioning 25 - patients. Intraparenchymal 48 people, extraorganic lesion 47 patients. There 262 with pNETs who underwent various surgeries Various resections performed 167 (63.8%) cases, enucleations (36.2%) Traditional surgical approach was used 65 Pancreatic fistula occurred 21 (type...

Aim . To identify the most appropriate surgical treatment method for patients with colorectal cancer metastases to liver by means of a comparative analysis immediate and long-term outcomes anatomical parenchyma-sparing resections. Materials methods A retrospective was conducted on 87 who underwent surgery between 2008 2023. The involved pseudorandomization eliminate influence unaccounted factors improve statistical validity study. Age, sex, body mass index, synchronous nature metastasis,...
Solid pseudopapillary pancreatic tumor (SPT) is a rare highly differentiated malignant neoplasm associated with relatively favorable clinical course. Most frequently it met in 20–30 years old women. SPT has no specific manifestations. Radiologic diagnosis (US, CT, MRI) reveals characteristic for this features like heterogenicity and hypovascularicity, reflects solid cystic components of the neoplasm. Sporadic reports describe extraorgan location SPT. Main method treatment surgery. Extent...
Aim. Combined evaluation of liver functional and volumetric reserves prior to advanced resections. Material Methods. The study included 24 patients who underwent extended resections for the period 2013– 2016. Clinical laboratory data, indocyanine green (ICG) clearance, CT volumetry, static polypositional hepatoscintigraphy were assessed in all cases. Results. Individual tests showed sufficient reserve FLR patients. However 7 (29.2%) cases refused favor less volume removal, two-stage surgery...
Vascular leiomyosarcoma (LMS) is an extremely rare disease. Venous lesion approximately 5 times more common than arterial. LMS usually found in retroperitoneal veins and inferior vena cava. Lesion of the splenic vein rare. A 52-year-old woman with recurrent abdominal pain reported article. Abdominal contrast-enhanced CT revealed a 6 cm tumor pancreatic tail. The patient underwent laparoscopic distal pancreatectomy splenectomy en bloc. Histological immunohistochemical studies originating from...
The paper describes a clinical case of pancreaticoduodenectomy for pancreatic ductal adenocarcinoma with superior mesenteric vein resection without its reconstruction. This is the first study kind found in Russian-language literature. In total, 16 such observations have been described relevant world literature to date. A 74-year-old female patient was diagnosed head 1.1 × cm size adjacent vein. After four courses ineffective neoadjuvant therapy, tumor completely obliterated control...
Liver surgery constantly undergoes significant changes and is greatly influenced by scientific technological developments. At present, liver resection gives hope for the patient's recovery. New tendencies in include parenchyma-sparing surgery, minimization of margin, skeletonization blood vessels. The principle along hepatic veins with specifically detailed landmarks currently being introduced. This allows surgeons to expand their capabilities covering new cohorts patients multiple tumors,...

The paper describes a clinical case of successful combined modality treatment patient with pancreatic ductal adenocarcinoma extension to the superior mesenteric artery and vein. examination revealed an unresectable tumor. Twelve courses FOLFIRINOX therapy showed good results, upon which plan was changed typical pancreaticoduodenal resection performed. Pathological urT1N0M0R0, TRG 2. postoperative period uneventful. efficacy drug for has increased markedly in recent years. Neoadjuvant...

We present a clinical case of pancreatoduodenal resection with en bloc superior mesenteric vein (SMV) without reconstruction. This represents an infrequent situation. Only 16 such cases are reported in world literature and no similar Russian-language literature. A 74-year female patient was diagnosed pancreatic ductal adenocarcinoma. After detailed examination via computer tomography (CT) tumor-caused compression SMV found. 4 courses neoadjuvant chemotherapy, tumor has totally obliterated...
Aim. To study the effectiveness of developed method consisting crushing liver parenchyma by clamp connected to high-frequency generator with simultaneous irrigation dissection line sodium chloride solution. Material and Methods. We investigated results 40 resections which were divided into 3 groups. The group included 15 performed original method. first control consisted 18 bipolar coagulation, second 7 harmonic scalpel. Results. Blood loss was 627.011 ± 161.894 ml, 811 ml 225.088 1652.29...
The article presents clinical case of successful treatment patients with metachronous, resectable liver metastasis colorectal cancer located in hepatic veins confluence. It is shown the results consecutive chemotherapy and portal vein embolization which reduced size increased volume left lateral sector. Readiness for transplantation has allowed to perform radical R0-resection. We obtained satisfactory long-term outcomes.
